Șizofrenide bilișsel onarım tedavisi

Son yıllarda şizofreni tedavisinde yaşanan gelişmeler sonucu şizofreni belirtilerinde iyileşme artık farmakolojik tedaviler ile ulaşılabilir bir
hedef haline gelmiştir. Bu gelişmelere rağmen, şizofreni hastalarında yıllar içerisinde yıkıma uğrayan ve sosyal işlevsellik üzerinde önemli
bir öngörücü olan nörobilişsel yetiler için aynı başarı söz konusu değildir. Önceki çalışmalarda, nörobilişsel kayıpların mevcut konvansiyonel
tedavilere yanıtının düşük düzeyde olduğu gösterilmiştir. Bu nedenle araştırıcıların nörobilişsel yetileri iyileştirebilecek psikolojik tedavi
uygulamalarına olan ilgisi artmıştır. Şizofrenide yıkıma uğrayan bilişsel yetileri onarmayı ve dolaylı olarak sosyal işlevselliği geliştirmeyi
hedefleyen uygulamalara bilişsel onarım terapileri (BOT) adı verilir. Son yıllarda artan bir ivme ile üzerinde çalışılan BOT uygulamalarının,
nörobiliş, belirti şiddeti ve sosyal işlevsellik üzerindeki olumlu etkileri kanıt düzeyi yüksek meta-analizlerde gösterilmiştir. Günümüzde, bilgisayar
veya kağıt kalem egzersizleri ile “öğren ve pratik yap” temeline dayalı BOT uygulamaları yaygın kullanımdaki psikososyal tedavilere
entegre edilmeye ve rutin tedavi kılavuzlarında yer almaya başlanmıştır. Tüm bu gelişmelere rağmen, ülkemizde henüz şizofreni hastalarına
uygulanmakta olan bir BOT programı bulunmamaktadır. Bu makalenin hedefi; ülkemiz koşullarında uygulama kolaylığı açısından önem arz
eden şizofrenide BOT uygulamaları hakkında tanım, iyileştirmeyi hedeflediği nörobiliş alanları, etkinliği, uygulama teknikleri, tedavi modeli
ve kısıtlılıkları içeren bir literatür gözden geçirmesi sunmaktır. Bu amaç ile PsycINFO ve MEDLINE/PUBMED veri tabanlarındaki şizofrenide
bilişsel onarım alanında yapılan kanıt değeri yüksek çalışmaların ve meta-analizlerin verileri gözden geçirilmiş, alanın öncülerinin
görüşleri ile harmanlanarak sunulmuştur.
Cognitive remediation therapy in schizophrenia
considering the recent developments in psychopharmacological treatments, nowadays symptomatic remission has become a realistic target
for treatment response in schizophrenia. Nevertheless, this issue is far from being the case for neurocognitive deficits which are closely linked
to the social functioning in schizophrenia. Previous studies found small effect sizes of conventional treatments on improving neurocognition
in schizophrenia, hence an extensive need has emerged for developing novel psychological treatments for tackling with neurocognitive
deficits and improving social functioning in schizophrenia. Cognitive Remediation Therapy (CRT) is the so-called psychological treatment
which consists of several techniques from behaviour therapy, learning psychology and neuropsychology for remediating the neurocognitive
deficits in schizophrenia. In the recent years, a substantial number of studies found promising effects of CRT programs on improving various
domains such as neurocognition, symptom severity and social functioning for schizophrenia patients. Recent efforts are along the lines of
combining the CRT programs which are based on “learn and practice” by using either a computer interface or paper and pen exercises, with
well-known psychosocial treatment approaches in schizophrenia. Some of these programs have already been considered as possible options
in the recent treatment guidelines. Up until now, this important treatment option was somehow neglected in Turkey. Therefore the aim of
this article is to review the on-going work about CRT programs and introduce the basics of CRT to the reader. Specifically, the main focuses
of this work are the neurocognitive targets oftreatment, effectiveness, treatment techniques and models, future aspects and limitations of
CRT in schizophrenia. The recent and fundamental works in the field of CRT in schizophrenia were collected by a literature search on
PsycINFO, and MEDLINE / PUBMED data bases. These previous studies were combined with the opinions of the pioneers of the area
and are presented.
