Şizofreni ve aile arasındaki ilişkiye tarihsel gelişimi açısından bakıldığında psikolojik kuramı savunan çoğu araştırmacının aileyi hastalığın
önemli bir nedeni olarak kabul ettiği görülmektedir. Ruh sağlığı çalışanlarıyla ailelerin ilişkisine fazlasıyla zarar vermiş olan söz konusu
kuramlar günümüzde geçerliliğini yitirmiştir. Biyolojik ve çevresel etkenleri bir arada değerlendiren yatkınlık-stres modeli şizofreninin
gelişimini ve yinelemeleri açıklamada bir kavrayış sağlamaktadır. Aile tedavi planına dahil edilmelidir. İlk psikotik atak ya da akut alevlenme
nedeniyle hastaneye yatış, ailelerle bağlantı kurmak için uygun dönemlerdir. Ailelere suçlamadan, destekleyici ve eşduyuma dayalı bir tutumla
yaklaşılmalıdır. Aileye yönelik girişimlerin çoğu “ruhsal-eğitim” başlığı altında yer alabilmekte ya da ruhsal-eğitim söz konusu girişimin
özünü oluşturmaktadır. Aileye yönelik girişimler aile üyelerini hastalıkla ilgili olarak bilgilendirmeyi; stres etkenlerini azaltma ve hastalığın
neden olduğu zor ve stresli durumlarla daha etkin biçimde başa çıkma yöntemlerini öğretmeyi içermelidir. Depreşme ve yeniden hastaneye
yatış oranlarını azaltan ruhsal-eğitimsel girişimler kapsamlı bir psikososyal tedavi programının ayrılmaz parçası olmalıdır. Bunun yanı sıra
aileler sivil toplum örgütlerine katılmaları konusunda desteklenmelidir.
The family interventions in schizophrenia
When the relationship between schizophrenia and family is investigated in a historical context, it is seen that most investigators supporting
psychological theory consider the family to be the cause of the disease. These theories, which have damaged the relationship between
the clinician and the family members, have lost their validity during the last forty years. The development of vulnerability-stress models
that incorporated both biological and environmental factors is explaining both the development of schizophrenia and the occurence of
subsequent episodes. Family should be included in the treatment plan. Hospitalization due to the first psychotic episode or further psychotic
recurrences is the most convenient periodfor establishing contact with the families. One should approach the family in a supportive and
empathic manner, without putting any blame on them. Most of these family intervention programs can be subsumed under the category of
“psychoeducation” or at least contain psychoeducation as an essential component. Interventions programs should provide information about
schizophrenia to all family members; teach methods of decreasing environmental stress; help to cope more effectively with difficult and
stressful situations associated with the illness. The psychoeducational family interventions reduce the relapse and re-hospitalization rates of
schizophrenic patients. Psychoeducation for families should become a basic part of a comprehensive psychosocial treatment package that
is offered to all schizophrenia patients. In addition to, families should be supported in terms of joining nongovernmental organizations.
