Modern psikiyatrik tanı sınıflandırma sistemlerine göre konversiyon bozukluğu, psikolojik çatışmaya eşlik eden, fiziksel bozukluğu düşündürecek şekilde işlevsellikte kayıplarla giden, bir ya da daha fazla nörolojik belirti ile belirli bir bozukluk olarak tanımlanmaktadır. Konversiyon bozukluğu DSM-IV-TR sınıflandırma sisteminde somatoform bozukluklar tanı grubunda yer almaktayken, ICD-10 tanı sisteminde dissosiyatif bozukluklar tanı grubunda bulunmaktadır. Bazı yazarlarca yayınlanması beklenen DSM-V sınıflamasında da konversiyon bozukluğunun ICD-10'daki gibi dissosiyatif bozukluklar grubunda yer alması gerektiği belirtilmiştir. Konversiyon bozukluğu tanısı konanların %25-50'si nde daha sonra nörolojik veya psikiyatri dışı bir hastalık ortaya çıktığı ifade edilmektedir. Bu yazıda ağırlıklı olarak motor belirtilerle başvuran ve belirtileri nörolojik bir hastalıkla açıklanamadığı için psikiyatri servisine yönlendirilen ancak klinik izlemi sırasında ölen bir olgu sunulmuştur. Geriye dönük olarak yapılan incelemesinde nöroloji kliniğince ölüm sebebi muhtemel otonomik işlev bozukluğu olarak düşünülen olgu gözden geçirilerek tartışılmıştır.

According to the modern psychiatric classification systems, conversion disorder is defined as a disabling disorder with one or more neurological symptoms that suggest a physical disease and accompanying to psychological conflict. DSM-IV-TR subsumes conversion disorder under somatoform disorders category, whereas, ICD-10 classifies it under dissociative disorders diagnoses. Some authors have argued that conversion disorder should be classified under the category of dissociative disorders in DSM V as in ICD-10. It is suggested that some neurological and general medical conditions eventually develop in 25-50 percent of the patients with conversion disorder diagnosis. In this paper, we report and discuss a case who has initially presented with predominant motor symptoms and was referred to psychiatry clinic given the reason that her symptoms were not explained with any neurological disorder, however, the patients died after her follow-up period. In her retrospective analysis, the neurology clinic considered that the patient might have suffered from autonomic dysfunction.
