Amaç: Çocuk ve ergenlerdeki Obsesif-Kompülsif Bozukluk (OKB) etyolojisinde farklı bilişsel ve nöropsikolojik özelliklerin bulguların ortaya çıkmasında ve kalıcı olmasında etkili olduğu iddia edilmektedir. Bu çalışma ile amacımız, tedavi almayan çocuk ve ergen OKB'u olan olgularla, sağlıklı kontrollerin olası nöropsikolojik ve bilişsel işlev farklılıklarının karşılaştırılmasıdır. Yöntem: Yaşları 8 ile 16 arasında değişen OKB'u olan çocuk ve ergenler (n=45) ve cinsiyet, yaş, el tercihi bakımından eşleştirilmiş kontrol grubu (n=45) çalışmaya alındı. Gruplar WISC-R, Wisconsin Kart Eşleme Testi, Stroop Testi TBAG formu, İz Sürme Testi A ve B bölümleri, Sözel akıcılık testi, Kategori çağrışım testi, Kategori değişim testi, Rey işitsel sözel öğrenme ve bellek testi ile karşılaştırıldı. Bulgular: Toplam zeka bölümü katsayıları benzer olsa da, kontrol grubuna göre OKB grubunda daha yüksek sözel zeka bölümü ve daha düşük performans zeka bölümü katsayıları tespit ettik. OKB'u olan çocuk ve ergenlerde Görsel-uzamsal-mekansal algılama, görsel tarama, anlık, kısa süreli sözel bellek ve öğrenme alanlarında gerilik saptadık. Uzun süreli sözel belleklerini ise daha güçlü tespit ettik. Sözel akıcılık, dikkat, çalışma belleği, tepki ketlemesi, bozucu etkiye direnç, bilgi işleme hızı, psikomotor hız, kurulumu değiştirme (set shifting), kavramsallaştırma, soyut düşünme ve irdeleme açısından gruplar açısından belirgin farklılık saptayamadık. Sonuç: Pediatrik OKB'da bazı bilişsel zayıf ve güçlü alanlar bulguların ortaya çıkışında etkili olabilir.

Objective: It is argued that different cognitive and neuropsychological features are involved in the presentation and permanency of symptoms in children and adolescents with obsessive-compulsive disorder (OCD). The aim of this study was to explore possible cognitive and neuropsychological variations between treatment-naive children and adolescents with OCD and healthy controls. Methods: Children and adolescents with OCD aged between 8 and 16 [year]s (n=45) and healthy controls matched for age, sex and hand preference (n=45) were recruited for the study. The groups were compared with the WISC-R, Wisconsin Card Sorting Test, Stroop Test-TBAG form, Trail Making Test A/B, verbal fluency and category fluency tests, category alteration test and the Rey auditory verbal learning test. Results: Despite the similar full-scale intelligence quotient (IQ) scores in the two groups, OCD group had significantly higher verbal IQ and lower performance IQ. We found weakness in visuospatial perception, visual scanning, immediate verbal memory and immediate verbal learning as well as cognitive strength in long-term verbal memory in OCD group. We could not detect any statistical differences in verbal fluency, attention, working memory, response inhibition, interference control, speed of information processing, psychomotor speed, set shifting, concept formation and abstract reasoning between the two groups. Conclusion: Some cognitive-neuropsychological weaknesses and strengths could be involved with presentation of OCD symptoms in children and adolescents.
