Depresif dönem sırasında başlanan ilk ilaçla tam iyileşme görülmeyebilir; bu durum da hastalık dönemlerinin sıklaşmasına, hastalığın
şiddetlenmesine ve ciddi yeti yitimine yol açar. Dirençli depresyon tedavisinde pek çok farmakoterapi yaklaşımı vardır. Mevcut ilaç uygun

etkinliği ile ilgili yeterli kanıt bulunmayan deneysel ajanlar da yer alır. Bunları dopaminerjik ajanlar, psikostimulanlar, nörohormonlar,
glutaminerjik ajanlar ve alternatif tıp yöntemleri olarak özetleyebiliriz.

Initial treatment is often insufficient to achieve full remission in a given depressive episode, resulting in more frequent episodes, worsened
severity, and major disability. There are several approaches to the pharmacotherapy of treatment resistant depression. If the optimization
of the current treatment fails, switching within or between classes of medication or augmenting the primary drug with another
drug may be tried. For the augmentation, medication with more evidence for their efficacy or novel agents is used. These novel agents
are dopaminergic agents, psychostimulants, neurohormones, glutaminergic agents or complimentary drugs are used. There is not enough
evidence to support the efficacy of these novel drugs.
