Ruhsal hastalıkların gelişiminde ve önlenmesinde sosyal destek sistemlerinin ilişkisi çok yönlüdür. Sosyal destek sistemlerinin en iç halkası olarak ailenin önemi ülkemizde psikiyatrik
hastalık tanısı olan kişilerin neredeyse tamamının ailesi ile yaşadığı bilgisi ile kavranabilir. Aile
içindeki yaşam ortamının psikotik bozuklukların seyrinde etkisi ile ilgili çalışmalar 1980'lerden itibaren duygu dışavurumu kavramı ile başlamıştır. Duygu dışavurumunun yüksek oluşu aile içinde sözel- duygusal şiddetin yaşandığını gösteriyor olabilir ve aile içi şiddetin diğer tiplerinin
varlığının sorgulanmasını gerektirir. Literatürde travma ile psikoz ilişkisini ortaya koymaya yönelik çalışmalara ilgi son yıllarda giderek artmıştır. Aile içi şiddet sıklıkla tekrarlayıcı doğada olan bir travma nedenidir. Eş- partner şiddeti ve çocukluk döneminde istismara maruz kalma ile psikotik semptomların ortaya çıkışını ve psikotik bozuklukların kliniğinin etkileşimini ele alan çalışmalar son
20 yılda giderek artmıştır. Psikotik bozukluğu bulunan kişilerde geriye dönük olarak şiddete maruz
kalmanın araştırıldığı çalışmalarda normal popülasyona göre daha yüksek oranlar saptanmıştır.
Bu gözden geçirme yazısında aile içi şiddet, eş-partner şiddeti ve çocukluk dönemi istismar ile
psikotik bozuklukların ilişkisini inceleyen çalışmalar ele alınmış ve sosyal ve toplumsal bir olgu
olan aile içi şiddetin yol açabileceği nöropsikolojik sorunlar arasında yer alan psikotik bozukluklar
açısından sonuçları karşılaştırılmaya çalışılmıştır. Bu çalışmalardan elde edilen sonuçlar travmaya
maruz kalma ile psikozun gelişimi arasındaki ilişkiye ışık tutmakta, nörogelişimsel aksamaların hangi
basamaklarda ve nasıl olabileceği hakkında yeni bilgi ve fikirler sağlamaktadır. İnsan eliyle yaratılan
önlenebilir ancak sıklıkla tekrarlayıcı nitelikte bir travma nedeni olarak aile içi şiddetin psikotik
bozukluklar ile ilişkisini araştıran ileri çalışmalar, ruhsal hastalıkları önleyici sağlık hizmetleri açısından ve sosyal destek sistemlerinin güçlendirilmesi bakımından önemlidir

Social support systems have various effects on development and prevention of psychiatric illness. Family is the inner core of social support systems and its
importance in Turkey can be understood through the knowledge that almost all of the people
with a psychiatric diagnosis live with their families in our country. The studies about the effect of
family environement on the course of psychotic disorders begin with the concept of ?expressed
emotion? in 1980's . The presence of high expressed emotions in the family might indicate
the presence of emotional and verbal violence and should initiate to look after other forms of
domestic violence. There is an increasing attention on studies evaluating the relation of trauma
with psychosis. Domestic violence is a cause of trauma which is mostly in repetetive nature.
Studies on relation and interaction of spouse-partner violence, or childhood abuse and the
emerging psychotic symptoms and the course of psychotic disorders have increased in the last
two decades. The studies about previous exposure to domestic violence among persons with
a diagnosis of psychosis reveal a higher prevelance than the normal population. In this review
article we evaluated studies about the relation between psychotic disorders and domestic
violence, spouse-partner violence, childhood abuse; we tried to compare the results in a broad
pserspective for domestic violence as a social problem and in regard to psychotic disorders
which might be one of the neuropsychological disturbances that domestic violence may cause.
The results obtained from these studies lighten the interaction between exposure to trauma and
the development of psychosis and supply new data and thinking in which pathways and how the
neurodevelopmental disturbances might occur. Further studies about domestic violence-a widely repetetive cause of human made trauma that can be prevented-are important for enhancing the social support systems and the health care systems about prevention of psychiatric illness.
