Otistik Bozuklukta Yeni Tedavi Arayışları. Gastrointestinal Sistem

Amaç: 1990’lı yıllardan itibaren otistik bozuklukta gastrointestinal sistemin rolüne yönelik pek çok çalışma
yürütülmüş, etyolojiye yönelik çalışmalar yeni tedavi arayışlarını da beraberinde getirmiştir. Bu yazıda
otistik bozukluk tanısı bulunan çocuklarda gastrointestinal sisteme yönelik tedavi yaklaşımları ile
ilgili yazın bilgileri gözden geçirilmeye çalışılmıştır. Yöntem: Otizmde gastrointestinal sistemle ilgili tedavi
uygulamaları konusunda PubMed arama motoru kullanılarak İngilizce olarak sunulmuş yayınlar
taranmış ve değerlendirilerek tartışılmaya çalışılmıştır. Sonuçlar: Otizmde gastrointestinal sistemle ilgili
tedavi uygulamaları gluten ve /veya kazeinden yoksun diyet, sekretin, ketojenik diyet, rejim, probiyotikler/
antifungal ajanlar/mayadan yoksun diyet, özel karbonhidrat diyeti, sindirim enzimleri, vankomisin
ve famotidin olarak saptanmıştır. Tartışma: Bu alandaki pek çok çalışmaya rağmen, otizmde etkinliği
kanıtlanmış bir gastrointestinal tedavi yaklaşımı bulunmamaktadır. Bu nedenle, otizmde gastrointestinal
sistemin rolü ve tedavi etkinliklerine yönelik iyi planlanmış çalışmalara ihtiyaç duyulmaktadır.
New Therapeutic Pursuits On The Autistic Disorder: Gastrointestinal System
Objective: Since 1990s, many studies have been conducted about the role of the gastrointestinal system
in the autism, and the studies on the etiology have triggered new therapeutic approaches. In this
paper, we tried to to review literature findings on the treatment modalities associated with gastrointestinal
system in autistic children. Method: Articles about gastrointestinal treatment modalities on autism
was searched from PubMed database for English [language], and selected articles were reviewed and discussed.
Results: Gluten-free/casein-free diet, secretin, ketogenic diet, fasting, probiotics/antifungal
agents/yeast-free diet, specific carbohydrate diet, digestive enzymes, vancomycin and famotidin were
found as the gastrointestinal treatment modalities on autism Discussion: Despite all the efforts, effectiveness
of any gastrointestinal treatment modality has not been shown on autism. So, we still need welldesigned
clinical studies on the relationship of gastrointestinal system and autism.
