Kronik Hasta Servislerinde Uzun Süre Yatan Şizofreni Hastaları Nasıl Tedavi Ediliyor?

Amaç: Bu çalışma ile Bakırköy Ruh ve Sinir Hastalıkları Hastânesi kronik psikiyatrik hasta servislerinde uzun süredir tedavi görmekte olan şizofreni tanılı hastaların nasıl tedavi edildiklerinin araştırılması amaçlanmıştır. Yöntem: Kronik psikiyatrik hasta servislerinde şizofreni tanısı almış 296 hastanın dosyaları geriye dönük olarak taranmış, hastaların sosyode-mografik özellikleri, antipsikotik kullanım biçimleri ve ilâç kombinasyonları incelenmiştir. Bulgular: Yatış süreleri 1 ilâ 36 yıl (16.0±10.2 yıl) arasında değişen, şizofreni tanısı konmuş 296 hastanın, tekli veya çoklu olarak 13 farklı antipsikotik kullandığı, klâsik antipsikotiklerden en fazla haloperidol ve klorpromazin kullanılmakta iken, atipik antipsikotiklerden en fazla klozapin'i n verildiği, depo antipsikotiklerden en çok şufenazin'i n tercih edildiği, 23 (%7.8) hastanın antipsikotik kullanmadığı görüldü. Antipsikotik tedavi altındaki 273 hastanın 144'ü nün (%48.8) tekli antipsikotik, 129?unun (%43) çoklu antipsikotik aldığı görülmektedir. Bunların içinde ikili kombinasyon olarak en sık haloperidol ve klorpromazin kombinasyonuna rastlanırken, 29 hastanın (%10) üçlü antipsikotik ilâç aldıkları dikkati çekmiştir. 42 (%14) hasta atipik antipsikotik (AAP) veya klâsik antipsikotiklerle (KAP) birlikte depo antipsikotik (DAP) kullanmıştır. 208 (%70.3) hasta bir veya birden fazla klasik antipsikotik, 108 (%36.5) hasta bir veya birden fazla atipik antipsikotik kullanmaktayken, 40 (%13.5) hasta bir ya da birden fazla klâsik ve atipik antipsikotiği birlikte kullanmaktaydı. 209 (%70.6) hasta antikolinerjik kullanmakta iken 49 (%16.6) hasta duygudurum dengeleyici veya antikonvülsan, 8 (%2.7) hasta antidepresan, 53 (%17.9) hasta benzodiyazepin kullanmaktaydı. Tartışma ve Sonuç: Ülkemizin en büyük ve en eski Ruh Sağlığı Hastânesi'nde ortalama 16 yıldır yatırılarak tedavi gören şizofreni tanılı hastaların tedavi dökümleri, hastaların tedavi şekilleri hakkında önemli bilgiler vermektedir. Bu çalışmanın daha da geliştirilerek, özellikle ilâçların etki ve yan etkilerinin ölçümünün yapılması, KAP ve AAP'lerin bu yönden karşılaştırılmasıyla daha fazla veri elde edilecektir.
How are the Treatments of Patients with Schizophrenia Hospitalized for a Long Time in the Chronic Inpatient Units?
Objective: With this study , the evaluation of their treatment of patients with schizophrenia who have been hospitalized for a long time in the chronic psychiatric patients services in Bakirkoy Research and Training Hospital for Psychiatry, Neurology and Neurosurgery was aimed. Method: The medical records of 296 patients with schizophrenia in the chronic psychiatric inpatient units have been retrospectively and sociodemographic properties, antipsychotic prescriptions and medication combinations of these patients have been examined. Findings: The duration of hospitalization of the 296 patients with schizophrenia were ranging between 1 and 36 [year]s (16.0±10.2 [year]s). They were using 13 different antipsychotics prescribed as monotherapy or in combination with another antipsychotic. Among the atypical antipsychotics (AAPs) clozapine was the most preferred one whereas fluphenazine was the most prescribed among depot antipsychotics and haloperidol and chlorpromazine were the most used among classic antipsychotics (CAPs). Additionally 23 patients (7.8%) were not using any antipsychotic medications. Of the 273 patients who underwent antipsychotic medication, the 144 (48.8%) were taking antipsychotic monotherapy and 129 (43%) were on a combination therapy with two or more antipsychotic drugs. The most frequent combination in the latter group was haloperidol and chlorpromazine combination. It was also interesting that 29 patients (10%) were under triple antipsychotic medication. 42 patients (14%) were using depot antipsychotic in combination with AAPs or CAPs. 40 patients (13.5%) were using one or more CAPs or AAPs together whereas 209 patients (70.3%) were using one or more CAPs and 108 (36.5%) patients were using one or more AAPs. 49 (16.6%) patients were using mood stabilizers or anticonvulsants, 8 (2.7%) patients were using antidepressants, and 53 (17.9%) were using benzodiazepines whereas 209 (70.6%) patients were using anticholinergics. Discussion and Conclusion: The examination of the treatment regimens of the schizophrenic patients who have been hospitalized 16 [year]s in average in the biggest and the oldest mental disorder hospital in our country provided us important data on this topic.
