Şizofrenik Hastalarda Negatif ve Pozitif Belirtiler ile Yapısal Beyin Görüntüleme (MRG) Bulgularının İlişkisi

Amaç: Bu çalışmanın amacı, şizofrenide pozitif ve negatif belirtilerle yapısal beyin patolojileri arasında bir ilişki bulunup bulunmadığını araştırmaktır.
Yöntem: DSM-IV ölçütlerine göre şizofreni tanısı konmuş, yaş ortalaması 44.7±15.9, eğitim süresi ortalaması 10.8±3.8 yıl, hastalık süresi ortalaması 18.9±13.1 yıl olan 8 kadın, 14 erkek, toplam 22 hastaya, sosyodemografik bilgi formu, Negatif ve Pozitif Belirtileri Değerlendirme Ölçekleri (NBDÖ, PBDÖ) uygulanmış ve hastaların tümünde MRG incelemesi yapılmıştır. Bağıntıların incelenmesinde spearman korelasyon analizi yöntemi kullanılmıştır.
Bulgular: Olgularda ortalama PBDÖ toplam puanı 36.3±22.4, NBDÖ toplam puanı 50.0±17.5 bulunmuştur. Yapısal beyin bulgularının küresel değerlendirmesi sonucunda hastaların 9�unda (% 40.9) kesin patoloji, 8�inde (% 36.4) şüpheli patoloji saptanmış, 5 olguda ise (% 22.7) patoloji belirlenmemiştir. Pozitif belirtilerle lateral, 3. ve 4. ventrikül, temporal ve parietal korteks, medial temporal yapı patolojileri arasında; negatif belirtilerle de lateral, 3. ve 4. ventrikül, medial temporal yapı ve bazal ganglia patolojileri arasında orta düzeyde, aynı yönde bağıntılar saptanmıştır.
Tartışma: Sonuç olarak, şizofrenik hastalarda temporal ve parietal kortikal patolojilerle pozitif belirtiler, ventriküler sistem ve medial temporal yapı patolojileriyle de hem pozitif hem de negatif belirtiler arasında bağıntılar olduğu belirlenmiştir. Bu sonuçlar; şizofrenide yapısal beyin değişimlerinin, pozitif ve negatif belirti boyutlarını birbirinden ayırt etmekte yararlı olabileceğini düşündürmektedir.
Correlations of Negative and Positive Symptoms with Brain MRI Findings in Schizophrenia
Aim: The aim of this study was to investigate the correlations between positive and negative symptoms and structural brain abnormalities in schizophrenia.
Method: 22 patients, 14 male and 8 female who met DSM-IV criteria for schizophrenia were included in this study. Mean age was 44.7±15.9 [year]s, duration of education was 10.8±3.8 [year]s and duration of illness was 18.9±13.1 [year]s. Demographic variables were obtained. Scale for the Assessment of Negative Symptoms (SANS) and the Scale for the Assessment of Positive Symptoms (SAPS) were administered. Magnetic resonance brain imaging was conducted in all patients. Obtained variables were evaluated by using spearman correlation analysis.
Results: Average SAPS total score was 36.3±22.4 and SANS total score was 50.0±17.5. After global evaluation of brain abnormalities, definite pathologies were found in 9 (40.9 %) of patients, questionable pathologies were found in 8 (36.4%) of patients. In 5 (22.7 %) patients no pathologies were detected. Medium degree positive correlations were detected between positive symptoms and lateral, third and fourth ventricles, temporal and parietal cortex and medial temporal structural pathologies; and between negative symptoms and lateral, third and fourth ventricles, medial temporal and basal ganglia structural pathologies.
Discussion: These results suggest that, positive and negative symptom dimensions in schizophrenia might be related with different structural brain abnormalities.
