Bakırköy Ruh ve Sinir Hastalıkları Eğitim ve Araştırma Hastanesi 1. Nöroşirürji Kliniğinde Ocak 1994 ile Mart 2002 tarihleri arasında 14 lomber dejeneratif spondilolitezis olgusu ve 11 lomber spinal travma olgusu opere edildi. Hastaların 15'i kadın, 10'u erkek olup yaş ortalamaları (23-70) 45 idi. Hastaların 18'inde şiddetli, 7'sinde orta şiddetli bel ve bacak ağrısı vardı; bunlardan 16'sında da çeşitli oranlarda nörolojik defisit mevcuttu. Tüm hastalara posterior transpediküler vida fiksasyon ile posterolateral kemik füzyon ameliyatı yapıldı. Bu vakaların preoperatif, postoperatif ve uzun dönemdeki klinik ve radyolojik takipleri prospektif-retrospektif olarak incelendi. Cerrahi girişim kararı verilirken Denis'in spinal üç kolon modeline dayalı spinal instabilitenin radyolojik ve klinik bulguları esas alındı. Hastaların nörolojik durumları Frankel sınıflaması ve ASIA (modifiye Frankel skalası) temel alınarak değerlendirildi.
Takiplerde füzyon oluşumu için direkt radyografiler ve aksiyal BT incelemeleri kullanıldı. Yaptığımız bu çalışmada dejeneratif spondilolistezis ve spinal travmalı hastaların cerrahi tedavisinde, transpediküler vida fiksasyonu ile yeterli fiksasyon sağlandığı görüldü. Sonuç olarak; bu tür hasta gruplarında uygulanan trans-pediküler vida fiksasyonu ve posterolateral füzyon ile tatminkar sonuçlar alındığı ve bu nedenle imkan olan durumlarda tercih edilmesi gerektiği kanısına varıldı.

14 lumbar degenerative spondylolisthesis and 11 lumbar spinal trauma cases were operated at Bakirkoy Psychiatric and Neurologic Diseases Hospital, 1st Neurosurgery Clinic, Istanbul, between January 1994 and March 2002. 15 of the 25 cases were female and 10 were male and their ages were ranged between 23-70 (mean 45). The presenting symptoms were severe and mild low-back and leg pain in 18 and 7 cases respectively. 16 patients had additional neurologic deficits of varying degrees. All of the patients underwent posterior transpedicular screw-rod stabilization combined with posterolateral bony fusion. The pre-operative and post-operative clinical and radiological follow-ups of the series was evaluated in both retrospective and prospective manner.
Surgical indications were based on the Denis' spinal three columns model showing the degree and severity of the spinal instability. Frankel's classification and modified Frankel's classification (ASIA) scales were used to evaluate the severity of the neurologic dysfunction.
Direct radiographs and CT-scans were used to evaluate the cases in the follow-up periods and it was seen that by posterior transpedicular screew-rod fixation and posterolateral bony fusion, satisfactory fixation of the spinal column was achieved. In conclusion, we recommend posterior transpedicular screw-rod fixation combined with posterolateral bony fusion in degenerative spondylolisthesis and spinal trauma cases of the thoraco-lumbar spine.
