Amaç: Meme kanseri sıklığında ve sağkalım oranındaki artış, bu kadınların psikolojik tepkilerinin, psikiyatrik morbiditenin ve yaşam kalitesinin incelenmesinin de önemini artırmaktadır. Bu çalışmanın amacı, meme kanserli hastaların cerrahi tedavi öncesine göre cerrahi tedavi sonrası erken ve geç dönemde anksiyete, depresyon ve yaşam kalitesini değerlendirmektir.

Yöntem: Bu çalışma yeni tanı konulmuş meme kanserli 84 kadında gerçekleştirilen prospektif, iki merkezli bir çalışmadır. Hastaların yaşam kalitesi, anksiyete ve depresyon düzeyleri cerrahi tedavi öncesi (pre-op), cerrahi tedavi sonrası (post-op) 1-3 ay ve 9-12 ay arasında olmak üzere üç kez değerlendirildi. Verilerin toplanmasında hastanın sosyodemografik, tıbbi ve psikolojik durumuna ilişkin sorulardan oluşan yarı yapılandırılmış görüşme formu, Hastane Anksiyete Depresyon Ölçeği (HAD) ve Dünya Sağlık Örgütü Yaşam Kalitesi Ölçeği Kısa Formu Türkçe Versiyonu (WHOQOL-BREF TR) kullanıldı.

Sonuçlar: Hastaların yaş ortalaması 52.18±11.17 yıldır. %80'i ne hem cerrahi, hem adjuvan tedavi uygulandı. Hastaların ölçüm zamanlarına göre sırasıyla anksiyete oranı %33.3, %35.7, %28.6 ve depresyon oranı %40.5, %42.9 ve %44.0 bulundu. Anksiyete ve depresyon puanı ortalamaları açısından üç dönem arasında istatistiksel olarak anlamlı fark saptanmadı. Hastaların yaşam kalitesinin fiziksel, psikolojik, sosyal ilişkiler alanı, genel algılanan yaşam kalitesi ve algılanan sağlık durumu puanlarının hem post-op erken dönemde, hem de geç dönemde pre-op döneme göre düşük olduğu saptandı (p≤0.01).

Tartışma: Hastaların anksiyete, depresyon riskinin tanı aşamasından itibaren yüksek olduğu ve riskin birinci yılda da devam ettiği görüldü. Yaşam kalitesinin post-op erken dönemde azaldığı ve birinci yılda sosyal ilişkiler alanı dışında diğer alanların yükselmeye başladığı ancak birinci yılda hala pre-op döneme göre yaşam kalitesi düzeyinin düşük olduğu belirlendi.

Objective: Assessment of psychological reactions, psychiatric morbidity and quality of life of breast cancer patients gains importance because of the increase in breast cancer incidence and survival rate. The aim of this study was to examine anxiety, depression and quality of life in breast cancer patients during early and late postsurgical periods compared with presurgical period.

Method: 84 newly diagnosed breast cancer patients were included in this prospective, two-center study. The quality of life, anxiety and depression in these patients were examined three times: before surgical treatment, at 1st-3rd [month] and at 9th-12th [month] postsurgically. The Hospital Anxiety and Depression Scale (HAD), the WHO Quality of Life Scale (WHOQOL-BREF) and a form including questions about sociodemographic status, medical and psychological status of the patients were used.

Results: The mean age of patients was 52.18±11.17 [year]s. 80.0% of all patients had both surgical and adjuvant treatment. The anxiety rates were 33.3%, 35.7% and 28.6%, and depression rates were 40.5%, 42.9% and 44.0% with respect to the evaluation time. No statistical difference was detected between the anxiety and depression rates of the three periods. The results of physical, psychological, social and general parts of quality of life scale at both early and late postoperative periods were less than the results of preoperative period (p≤0.01).

Discussion: The risk of anxiety and depression in the patients was high starting from the diagnosis date and was still high one [year] after the surgical treatment. It was observed that, the quality of life decreased in early postsurgical period, and started to increase from the first [year] after surgery,, however, it was still lower compared to the presurgical period.
