Dünya Sağlık Örgütü sağlığı fiziksel ve ruhsal bir iyi olma hali olarak tanımlamaktadır. Bu tanımlamaya göre, madde kullanım bozukluğu (MKB) ruh sağlığını tehdit eden bir durumdur. MKB nedenlerini araştıran çalışmalarda, maddeyi ilk deneme sırasında çevresel koşulların önemi vurgulanırken, maddeyi yeniden kullanmada biyolojik yatkınlığın sorumlu olduğu belirlenmiştir. MKB bozukluğunda çevresel şartların ön planda olması ve sorunun tüm toplumu ilgilendirmesi, bu hastalıkla mücadelede koruyucu ruh sağlığı uygulamalarının önemi artmaktadır.

Toplumu, uyuşturucu ve uyarıcıların olumsuz etkilerinden uzak tutmada birincil korunma uygulamalarının hedefi maddeyi kullanmaya başlamamış, ya da henüz yeni denemiş kişilerdir. İkincil ve üçüncül korumalar tedavi ve esenlendirme programlarını kapsamaktadır.

Bugüne değin MKB ile mücadelede çok sayıda proje geliştirilmiş olmasına karşın, bunların çoğunda beklenen başarı elde edilememiştir. Bu başarısızlığın temel nedeni, çözümün yalnız tek bir boyutta düşünülmesidir. Bu toplumsal mücadele projeleri oluşturulmadan önce, politika, uygulama, eğitim, iletişim ve işbirliği konularında hazırlıklar yapılmalıdır. Bu beş başlığın eşgüdümü başarının temel anahtarıdır. Bu konudaki etkin projeler için farklı yöntem ve stratejiler arasında bir dengenin kurulması gerekmektedir.

Eğitimin bu başlıklar arasında tartışılmaz bir yeri vardır. MKB’de eğitimin yeri hem tedavi, hem de koruma boyutunda yıllar içinde değişim göstermiştir. Alkol bağımlılığı ilk olarak 1956 yılında New York Tıp Cemiyeti tarafından tıbbi bir sorun olarak kabul edilmiştir. Bu yılla kadar MKB’nin yasal ve ahlaksal bir sorun olduğu düşünülmekteydi. Sorunun bu boyutlarıyla ele alınması korunma stratejilerinin belirlenmesinde dinsel ve yasal uygulamaların ön plana geçmesine, tedavideyse tıbbi uygulamaların yeterince gelişmemesine neden olmuştur. 1920’li yıllarda narkotik bağımlıların tedavisinde opioid kullanan hekimlerin sanık durumuna düşmesi bu duruma bir örnek olarak verilebilir. Buna benzer olumsuz deneyimler, hekimlerin MKB tedavisinden kaçınmasına neden olmuştur. Günümüzde bile yeterli tıbbi tedavi alternatifinin olmaması, sorunun adli ya da ahlaksal nedenlere dayandırılması DSM-IV ölçütlerine göre MKB olanların ancak yüzde 7.5’inin tedaviye başvurmasına neden olmaktadır. 1970’de ABD Ulusal Alkolizm Danışma Kurulu toplantısının sonuç bildirgesinde daha iyi tedavi alternatifleri, esenlendirme programları ve korunma çalışmalarının geliştirilebilmesi için hekim ve diğer tıbbi personelin eğitilmesinin gerekliliği vurgulanmıştır. MKB ile mücadelenin başlangıç noktası yeterli donanımı olan uzmanlaşmış profesyoneller yetiştirmektir.

Profesyonel eğitimi ile bir bütünlük oluşturacak şekilde halk eğitimleri de doğru ve mantıksal kurgulara dayandırılmalıdır. Bu tasarımlar kapı maddesi olarak saptanan sigara ve alkolü de kapsamalıdır. Bu kapı maddeleri mortalite ve morbidite açısından toplum sağlığını tehdit etmektedir. Buna karşın, bazı çevreler bu maddeleri bir kişisel tercih, yasal-ticari ürün ve sorunsuz madde olarak değerlendirmektedir. Bu da maddelerin yasal olan ve olmayan gibi yanlış bir kategorik ayrımla sınıflandırılmasına neden olmaktadır. Oysa kafein dışında duygu-durumda dalgalanmaya neden olan maddelerin tamamının dağıtımı, kullanımı, satışı sınırlandırılmış ya da yasaklanmıştır; ancak halk arasında böyle bir değerlendirme yapılmamaktadır. Bu karmaşık durumun çözümü için 3 kategoriden oluşan alternatif bir sınıflandırma önerilebilir: Birinci kategori Sigara, alkol (Satışı ve kullanımı belli yaş sınırı üstünde yasal olanlar) İkinci kategori Psikoaktif maddeler (Reçete edilmediğinde satışı, kullanımı ve bulundurulması yasa dışı olanlar) Üçüncü kategori Esrar, kokain, eroin ve diğerleri.

Sonuç olarak MKB ile mücadelede her toplum kendi kültürel dinamikler göz önüne alarak evrensel bilgiler ışığında kendi modelini yaratmalıdır.

World Health Organization describes health as the state of being physically and psychologically good. According to this description substance abuse is a threat to psychological health. Based on the studies, which search the reasons of substance abuse, the importance of the environ-mental circumstances is highlighted at the initial experience stage whereas a biological factor is the reason at retesting. As the environmental circumstances play importance role in substance abuse and since this is a problem of whole society the importance of protective psychology applications increases while struggling with this addiction.

The people, who recently start abusing substance or recently try the substance, are the primary targets of prevention psychological applications in keeping society away from the negative impacts of substance. The secondary and third preventions include the therapy and rehabilitation programs.

There has been developed a number of projects about substance abuse until now. Despite this many of them have not been successful as expected. The primary reason for this failure is that solution has been taken within only one dimension. Before forming these social fray projects it is important to make preparations about politics, application, training, communication and cooperation. The coordination of these five topics is the basic key of achievement. This requires balancing the different methods and strategies for effective projects about this subject.

Education has an inevitable place among these topics. The substance abuse training has indicated a change in treatment and protection among the [year]s. Alcoholism has been accepted as a medical problem by New York Health Association in 1956. Until this [year] substance abuse has seen as legal and moral problem. Since the problem has been in this way, the religious and legal applications have played the primary role in the determination of protection strategies and as a result the medical applications could not have been developed enough in the treatment. For instance the doctors, who used opioid in the treatment of narcotic users, have been treated as suspects in 1920s. These types of negative experiences have caused the doctors stay away from substance abuse treatment. The medical treatment alternatives are limited even today and the problem is still based on legal or moral reasons. As a result of this only 7.5% of substance abusers ask for treatment according to the DSM IV. Better treatment alternatives, rehabilitation programs and the necessity of educating doctors and other medical personnel for developing the protection studies have been emphasized in the final report of USA Alcoholism Consultative Committee in 1970. Educating the equipped and specialized professionals is the starting point of struggling with this abuse.

The common training programs should be structured on real and logical basics in line with the professional programs. These concepts should also include cigarette and alcohol which are determined as gateway drug. These gateway drugs threaten the society’s health in terms of mortalite and morbidite. Despite this some see these substances as personal preference, legal commercial product and free of problem. This causes a classification of legal and illegal substances based on incorrect categorical differentiation. Whereas, the distribution, abuse, trade of all of the substances except caffeine are limited or prohibited. However, society has no this type of assessment and understanding. We propose an alternative classification consisting of 3 categories for the solution of this complex situation: first category: cigarette, alcohol (can be purchased and consumed by people over a certain age legal); second category: psychoactive substances (illegal without prescription); third category: Marihuana, cocaine, heroin, etc.
Each society should create its own model based on the cultural dynamics and in the light of universal information while struggling with substance abuse
