İntrakranyal Anevrizmaların Cerrahi Tedavisi ve Sonuçları

Amaç: İntrakranyal anevrizma yırtılmasına bağlı olarak gelişen subaraknoid kanama acil olarak müdahele edilmesi gereken bir hastalıktır. Ayrıca % 50 gibi yüksek bir mortalite oranına sahip katastrofik bir olaydır. Nöroşirürji kliniklerine ulaşabilen hastaların ancak % 60'ı normal hayatlarına devam edebilir, % 20'si ise kaybedilir. Bu nedenlerden dolayı anevrizma yırtılmasına bağlı olarak gelişen subaraknoid kanamalarda prognozu aydınlatabilecek öncül faktörlerin ve bunların sonuçlar üzerine etkileri ayrıntılı bir şekilde değerlendirilmelidir.
Materyal ve Yöntem: Bakırköy Prof. Dr. Mazhar Osman Ruh Sağlığı ve Sinir Hastalıkları Eğitim ve Araştırma Hastanesi III. Nöroşirürji Kliniği'nde Ocak 1996 ve Temmuz 2004 ayları arasında 130 olguda intrakranyal anevrizma tespit edilmiştir. Bu 130 olgunun 119'u (% 91.5) intrakranyal anevrizmanın kliplenmesi operasyonu yapılarak tedavi edilmiştir. Kalan 11 hasta ise coil ile embolize edilmiştir. Çalışmamızda olgularda yaş, cinsiyet, hipertansiyon varlığı, alkol, sigara kullanımı, anevrizma lokalizasyonu, giriş nörolojik durumu ve hidrosefali gelişiminin, hastaların çıkış nörolojik tablosu üzerine etkileri olan literatür ışığında tartışılmıştır.
Bulgular: Örneklemi oluşturan toplam 119 anevrizma olgusunun cinsiyet dağılımına bakıldığında 71 (% 59.7) kadın, 48 (% 40.3) erkek ve yaş ortalaması 47.3 yıl olarak bulunmuştur. Çalışmamızda mortalite % 10.1, hastaların % 67.2'sinin iyi nörolojik tabloya sahip olarak taburcu edildiği belirlenmiştir. Lineer Regression analizi stepwise yöntemi ile hastanın giriş nörolojik durumunun (Beta= -0.378, p<0.005) ve cinsiyetinin (Beta= -0.282, p=0.001) hastaların çıkış nörolojik tablosu üzerine istatistiksel olarak anlamlı etkileri olduğu tespit edilmiştir.
Sonuç: Diğer çalışmalara benzer olarak cinsiyet dağılımında kadın (% 59.7) hastaların daha fazla olduğu tespit edilmiştir. Ancak, farklı olarak kadın cinsiyeti olmanın prognozu kötü yönde etkilediğini belirledik. Benzer olarak da giriş nörolojik durumu kötü olan hastaların prognozunun kötü olduğunu tespit ettik. Çok merkezli ve geniş bir çalışma ile daha sağlıklı sonuçların elde edilebileceği düşüncesindeyiz.
Surgical Treatment of Intracranial Aneurysms and Outcomes
Objective: Subarachnoid hemorrhage which develops due to intracranial aneurysm rupture, is an urgent medical situation. It is also a catastrofic incident with a high mortality rate of 50 %. 60 % of the patients who can reach neurosurgery clinics can survive, whereas 20 % of them lose their life. Therefore, the primary factors and their effects on the final results should be overviewed in detail so they can shed light on the prognosis of aneurysm related subarachnoid hemorrhage.
Material and Methods: In Bakirköy Professor Dr. Mazhar Osman, Mental Health and Neurological Diseases Training and Research Hospital, 3rd Neurosurgery Clinic, 130 intracranial aneurysms diagnosed patients were admitted and observed between January 1996 and July 2004. One hundered and nineteen of these 130 cases were operated and treated by clipping the intracranial aneurysms. The remaining 11 patients were embolized with coil. In this study, the effect of age, gender, existence of hypertension, alcohol and tobacco consumption, aneurysm localization, hydrocephalus development and the patients state at the time of admission, to the neurological outcome of the patient, were discussed in the light of the literature.
Results: When we look at the distribution of gender within the total 119 aneurysms cases, we noticed that there were 71 (59.7 %) females, 48 (40.3 %) males and their mean age was 47.3. In our study, it is proved that 67.2 % of patients were found to have good neurological outcome at their time of discharge, and the mortality rate is 10.1 %. By using the linear regression analysis stepwise method, it is found that the neurological state (Beta= -0.378, p<0.005) and the gender (Beta= -0.282, p=0.001) of the admitted patient, have statistically significant effects on their neurological charts while being discharged from the hospital.
Conclusion: We noticed that female cases were 59.7 % predominant to mail in this case. This result was similar to other studies. However, we discovered that being female effected the prognosis in a deteriorating situation. Likewise, we discovered that patients with bad initial neurological states also had bad prognosis. We think that with a multi-centered and extensive study, a more sound result will be obtained.
