Hastane Başvurusu İntihar Girişimiyle Olan ve Olmayan Depresif Çocuk ve Ergenlerin Ayırt Edici Özellikleri

Amaç: Bu çalışmada intihar girişimiyle başvuran depresif bozukluklu hastalar (İGBDBH) ve intihar girişimiyle
hastane başvurusu olmayan depresif bozukluklu hastaların (İGHBODBH) ayırt edici özelliklerinin ortaya
konması amaçlanmıştır.
Yöntem: Çalışmaya yaşları 9-18 arasında çocuk acil servisine intihar girişimiyle başvuran depresif bozukluk
tanısı alan (n=44) ve daha önce intihar girişimi nedeniyle hastane başvurusu olmayıp çocuk psikiyatrisi
polikliniğine başvuran depresif bozukluk tanılı hastalar (n=47) alınmıştır. Sosyodemografik veriler, intihar
girişim özellikleri, stres faktörleri hazırlanan formlarla değerlendirilmiştir. Tüm hastalar ile DSM-IV kriterlerine
göre psikiyatrik değerlendirme yapılmıştır. İzlem sürecinde hastaların altı aylık tedavi uyumları değerlendirilmiştir.
Bulgular: İGBDBH’da daha fazla okulu bırakma (p= 0,013), parçalanmış aileden gelme (p=0.01), annelerinin
eğitimi olmama (p= 0,023) ve daha az ek psikiyatrik tanı aldıkları (p=0.008) gözlenmiştir. İntihar girişimcilerinde
intihar girişim nedeni olarak en fazla aile içi sorun (%59.1) belirtilmiştir. İGHBODBH’ın anneleri
daha fazla psikiyatrik destek ihtiyacı bildirmiş olup (p=0.006), izlemde daha fazla düzenli kontrole gelen
grup olmuştur (p=0.01).
Tartışma: İGBDBH’da komorbidite önemsiz, yaş, cinsiyet, okul ve aile özellikleri önemli değişkenler olarak
bulunmuştur. Ebeveynlerin gerekli psikiyatrik tedaviyle ilgili farkındalıklarının yüksek olması depresyon varlığında
çocuk psikiyatrisi poliklinik başvurularına neden olmakta ve gençleri intihar girişimiyle başvurudan korumada
önemli gibi görünmektedir.
Differential Features of Children and Adolescents with Depression who Attended to Psychiatry Clinic with or Without Suicide Attempt
Objective: In this study, we aimed to investigate the differantial factors of children and adolescents with major depressive disorder who admitted
to the clinic with or without suicide attempt.
Methods: The 9-18 [year]-old children with major depressive disorder who attended to emergency department with a suicide attempt (n:44),
and depressive children and adolescent who attended to child psychiatry outpatient clinic and who had no admision to hospital previously
because of suicide attempt (n:47) were included. Socio-demographic variables, suicide attempt properties and stres factors were explored
by prepared forms. The patients were assessed for any psychiatric disorder according to DSM IV criteria. In the following six [month]s, the patients
were evaluated in terms of treatment compliance.
Results: The depressive disorder who attended to emergency department with a suicide attempt group revealed higher rates of school discontinuation
(p:.013), higher rates of separated family (p:.001), and maternal illiteracy (p:.023) and lower rate of comorbid psychiatric disorder
(p:.019). Leading cause of suicide attempt was reported to be the conflict in the family environment (59.1%). More mothers in the depressive
disorder who admitted to the clinic without suicide attempt, beleived that their children needs a psychiatric treatment (p:.006), and
the compliance were significantly higher (p<.001).
Conclusions: Comorbidity seems to be not important but, age, gender, school and family features seems to be important variables in adolescents
with major depression who attended to clinic with a suicide attempt. Higher awareness of need for psychiatric treatment of the offsprings
according to mother revealed to admitted psychiatric clinic in depressive disorder and this awareness seems to protect adolescents
from attending to emergency clinic by suicide attempt.
