Tianeptin atipik bir antidepresandır. Trisiklik antidepresanlar ve selektif serotonin gerialım inhibitörlerinin (SSRI) aksine, santral sinir sisteminin serotonerjik sinapslarında serotonin geri alımını artırarak serotonerjik aktiviteyi azalttığı ileri sürülmüştür. Tianeptinin SSRI antidepresanlara zıt bir mekanizmaya sahip olduğunun iddia edilmesi depresyonun biyokimyasal temeli ve monoamin hipotezinin yeniden sorgulanmasını gerektirmiştir. Yakın tarihli bazı ilginç çalışmaların sonuçları da tianeptinin adenozin A1 reseptörleri üzerinden antikonvulsan aktivite gösterdiğine işaret etmiştir. Bu derleme makalede tianeptinin antidepresan etki mekanizması ve yeni santral etkileri güncel literatür ışığında değerlendirilmiştir.

Tianeptine is an atypical antidepressant drug. In contrast to tricyclic antidepressants and selective serotonin reuptake inhibitors (SSRIs), it has been suggested that tianeptine decreased serotonin’s activity and amount in serotonergic synapses of the central nervous system by increasing serotonin reuptake. Tianeptine, which has a mechanism of action opposite to that of SSRIs, necessitated a re-evaluation of the biochemical basis of depression and revealed that it cannot be explained solely by the monoamine hypothesis. In addition, recent interesting results indicating anticonvulsant activity of tianeptine and its possible interaction with adenosine A1 receptors were showed. In this review, antidepressant action mechanism of tianeptine and its novel central effects were evaluated in the light of the current literature.
