Amaç: Kronik böbrek yetmezliği organik kökenli bir hastalık olmakla birlikte psikolojik psikososyal sorunları da beraberinde getirir. Temel tedavisi diyaliz ve böbrek naklidir. Organ nakli olan bireylerin yoğun bir şekilde desteğe ihtiyaçları olmaktadır. Bu çalışmadaki amaç organ nakli sonrası olgulara destek sağlamak ve grup yaşantısı içinde ortaya çıkan temaları belirleyerek daha sonraki gruplara ışık tutmaktır. Yöntem: Bu çalışma 1994 yılında Gazi Üniversitesi Tıp Fakültesi Çocuk Psikiyatrisi Anabilim Dalı bünyesinde yapılan bir araştırmanın devamı niteliğindedir. Bu araştırmada diyaliz ünitesinde kronik böbrek hastalığı nedeniyle izlenmekte olan, yaşları 8-18 arasında değişen 4 erkek ve 1 kız ile bir etkileşim grubu çalışması yapılmıştır. Bu gençler benlik saygısı, durumluk ve sürekli kaygı, depresyon ve problem çözme ölçekleri ile değerlendirilmişlerdir. Bu gençlerde gelişim özelliklerinden dolayı ortaya çıkan kaygılar dışında hastalığa ilişkin kaygılarının yoğunlaştığı izlenmiştir. Ortaya çıkan tabloda gençlerin, ancak transplantasyon ile sorunlarının tamamıyla çözüleceği inancında oldukları görülmüştür. Bu gruba katılıp 1997 yılında böbrek nakil olan 1 kız ve 1 erkek hasta ile önceki gruba katılmamış ancak aynı dönemde böbrek nakil yapılan bir diğer hasta Gazi Üniversitesi Çocuk Psikiyatrisi Anabilim Dalı’nda 1998 yılında 15 hafta süren süren “transplantasyon sonrası yaşama uyum grubu” ile izlenmişlerdir. Sonuç: Olgular, aile, hastalık algısı, geleceğe ait beklentiler ve terapiste yansıtılan duygular bağlamında ele alınıp değerlendirilmiştir. Ortaya çıkan temalar güven duygusunun azalması, aileye uyum, geçmiş ve gelecekle ilgili kaygılar, ve organı n beden tarafından reddedileceği korkusu olarak belirlenmiştir. Bu alanlarda transplant olgularına destek sağlanmasının tedavinin başarısı açısından gerekli olduğu düşünülmüştür.

Objective: Chronic renal disease brings with it psychological and psychosocial problems although it has an organic etiology. Hemodialysis and kidney transplantation are the two major cures. This group of patients who are in risk of psychosocial problems need to be supported in order to enhance the success of treatment. The aim of this study was to provide support to organ transplant patients and to identify problem themes for this group during group therapy in order to help future groups. Method: This study is a follow-up of a study conducted in 1994 in Gazi University Medical Faculty Child Psychiatry Department. The researchers conducted group therapy with 4 males and 1 female aged between 8-18, who were receiving treatment at the hemodialysis unit due to chronic renal disease. These young people were administered self-esteem, state and trait anxiety, depression and problem solving inventories. It was observed that these young people have many worries due to their developmental period other than their illness. It was found that these children and adolescents thought that all their problems would be solved by a transplantation. One male and and 1 female who attended to the previous group and who had a transplant in 1997 as well as a male who did not attend the previous group yet who had a tranplant was followed by group therapy conducted in 1998 in Gazi University Medical Faculty Child Psychiatry Department. Results: The cases were assessed in terms of their families, perception of the illness, future expectations and feelings shown to the therapist. It was found that the problem areas were decreased self-esteem, adaptation to family, anxiety about the past and future and the anxiety of rejecting the organ. It is suggested that support should be provided to these patients in these areas.
