Depresyon tedavisi ve dolayısı ile farmakoterapisi yıllar içinde çok hızlı değişim gösteren bir alandır. Depresyonun tekrarlayan ve kronikleşebilen bir hastalık olması tedavi açısından daha fazla gelişmelere ihtiyaç duymamıza neden olmaktadır. Günümüzde depresyon tedavisinde akut dönem, sürdürüm ve idame dönemleri tedavisi gündeme gelmektedir. Akut dönem belirtilerin düzelmesi dönemini içerir. Çoğunlukla 6-8 haftalık dönemdir. Akut dönemden sonra gelen, tam ve devamlı düzelmeyi izleyen 16-20 haftalık dönem, “sürdürüm dönemi” olarak tanımlanır. Sürdürüm döneminde amaç major depresif dönemlerin depreşmesini engellemektir. İdamede amaç yineleme, intihar ve kronikleşmeyi engellemektir. Burada sürenin ne kadar ola-cağı konusunda yeterli veri yoktur. Proflaktik olarak da adlandırılan bu dönem konusunda klinik özelliklerine göre 3-5 yıl arasında süreler önerilmektedir. Temel olarak hiç bir antidepresanın diğerine üstünlüğü olduğunu söylemek mümkün değildir. Üstünlük daha çok yan etki ve uyum gibi faktörlere bağlı olarak ortaya çıkmaktadır. Bu makalede depresyon konusunda günümüzdeki mevcut tedavi ilkeleri gözden geçirilmiş, mevcut rehberlerden bazıları anlatılmış ve sonrasında bu rehberler ışığında depresyon tedavisi genel hatları ile tartışılmıştır. Tedavi süresinde bu kaynaklar ışığında düzenlemeler yapmak hem hasta hem de hekim açısından yararlı olacağı düşünülmektedir.

The treatment and especially pharmacotherapy of depression is changing very fast since past several [year]s. The recurrence and possible chronic nature of depression causes the necessity of improvement of treatment strategies. The treatment of depression includes three phases: acute phase, continuation phase and maintenance phase. The acute phase is the phase for remission of the symptoms. It’s usually 6-8 weeks. The aim of continuation phase is to decrease the likelihood of relapse. This phase comes after acute phase and lasts approximately 16-20 weeks. The goal of maintenance is to prevent recurrence, suicide and chronicity. It’s not obvious how long maintenance treatment should be continued. According to clinical features of the patient this phase can last between 3 and 5 [year]s. It’s shown that basically no antidepressants are superior to other antidepressants. The main superiority depends on the side effects and continuation of medications. In this review we have discussed the treatment principles of major depressive disorder, summarized some treatment algorithms. It’s in favor of the patient and the physician to use those algorithms during treatment.
