Nöroleptik Malign Sendrom ve Nöropsikiyatrik Sekelleri: Geriye Dönük bir Araştırma

Amaç: Bu çalışmada kliniğimizde, nöroleptik malign sendrom (NMS)
tanısı konulup takip edilen olguların klinik izlemleri ve sonrasında ortaya
çıkan nöropsikiyatrik sekellerinin incelenmesi amaçlanmıştır.
Yöntem: Ocak 2001 - Ocak 2006 dönemleri arasında Gaziantep Üniversitesi
Tıp Fakültesi Psikiyatri kliniğinde yatarak tedavi gören 870
hastanın dosyası araştırıldı. NMS tanısı konulmuş olan 10 olgunun tıbbi
dosyaları geriye dönük olarak incelenmiştir. Olguların tanıları DSM
IV tanı ölçütlerine göre konulmuş olup, olgular tedavi ve nöropsikiyatrik
sekeller yönünden değerlendirilmiştir.
Bulgular: NMS tanısı konulmuş 10 olgunun 4’ünde kalıcı nöropsikiyatrik
sekel geliştiği görülmüştür. Sekiz hastada NMS tedavisi sonrası 2
aylık dönem içerisinde devam eden dişli çark, rijidite gibi bulgulara
rastlanılmıştır. Hastaların uzun dönem izleminde ise 3 hastada bilişsel
kayıp, 1 hastada parkinsonizm ve 1 hastada elde fleksiyon kontraktürü
şeklinde kalıcı nöropsikiyatrik sekeller görülmüştür. Olgu 1’de ortaya
çıkan bilişsel işlevlerdeki kayıp 4 ay sonra normale dönmüş
olup, bilişsel işlev kaybının EKT uygulamasına bağlı olabileceği düşünülmüştür.
Diğer iki olguda gelişen bilişsel işlev kaybı kalıcı olmuştur.
Sonuç: Acil tedavi yaklaşımlarının geliştirilmesi ile NMS olgularının büyük
çoğunluğu iyileşmekte, ölüm oranları azalmaktadır. Bununla birlikte
bir kısım hastada kalıcı nöropsikiyatrik sekeller ortaya çıkabilmektedir.
Bu çalışma, NMS komplikasyonlarının yalnızca erken dönemle
sınırlı olmadığını göstermesi açısından önemlidir.
Neuroleptic malignant syndrome and neuropsychiatric sequelaes: A retrospective study
Objective: The aim of this study was to investigate the
neuropsychiatric sequelaes that occurred and persisted throughout
the follow up period of pateints who received a diagnosis of
neuroleptic malignant syndrome (NMS).
Method: The records of the 870 patients, who were hospitalized at
Psychiatric Hospital of Medical Faculty of Gaziantep University, were
evaluated retrospectively for possible diagnosis of NMS. Ten of 870
inpatients were found to receive a diagnosis of NMS according to
DSM-IV criteria during their hospitalization. The treatments and
development of any neuropsychiatric sequelae were investigated and
noted.
Results: Among 10 NMS cases, 4 of them seem to develop
neuropsychiatric sequelaes. Two [month]s after development of NMS,
in 8 patients, the signs of persisting cog-wheel rigidity and rigidity
were observed. During follow-up period of patients, 3 patients
presented cognitive deficiency, 1 presented parkinsonism, and 1
patient had flexion-contracture in his hand as a permanent
complication of NMS. The cognitive deficiency in case 1 resolved after
4 [month]s which had been presumed to be related with ECT
treatment. The symptoms of cognitive deficiencies in other cases did
not show any progress despite vigorous treatment.
Conclusions: With the development of emergency treatment
approaches, NMS cases usually respond to treatment and are mostly
improved mortality rates were decreased. However, some patients
may have persistent neuropsychiatric sequelaes which should be
taken into account during treatment course and should be dealt with
carefully.
