Obsesif Kompulsif Bozukluk (OKB) ve şizofreni, bilinen en eski psikiyatrik hastalıklardandır. Bu iki hastalık, altta yatan beyin mekanizma- ları, klinik görünüm ve tedavileri açısından belirgin olarak birbirinden ayrıdır. Bununla birlikte bu iki ayrı hastalığın bazı demografik, klinik özellikleri ve patofizyolojik temelleri ortaktır. Genel popülasyonda şizofreni prevalansı %1 ve OKB prevalansı %2-3 kadar olmasına rağmen şizofrenide obsesif-kompulsif semptomlar (OKS) ve OKB bu iki hastalığın rastlantısal olarak birlikte görülmesinden daha yüksek bir oranda görülür. Obsesyonlar ve sanrılar farklı fenomenolojik özellikler gösterdiği halde bazı vakalarda tam bir ayrım yapmak güç olabilir. OKB’yi psikotik bozukluklardan ayıran temel fenomenolojik özellikler; OKB’nin ego-distonik olması ve “içgörü”nün varlığıdır. Ancak DSM-IV ile birlikte OKB’de yetersiz içgörülü obsesyonların varlığı kabul edilmiştir. Ayrıca sanrıların obsesyonlarla ilişkili olabileceği ve obsesyonların sanrılara dönüşebildiği öne sürülmüştür. Şizofreni hastalarında psikotik içerikli obsesyonlara ilişkin bildirimler ve sanrılara yönelik içgörünün de deği- şik derecelerde görülebilmesi klinik ayrımı daha da karmaşıklaştırmaktadır. Buna uygun olarak, giderek artan kanıtlar, şizofreniden OKB’ye, içgörünün varlığı ve derecesinin belirlediği bir spektrumu işaret etmektedir. Son yayımlanan DSM kılavuzları, “zayıf içgörülü OKB” belir- tecinin varlığını kabul ederek, OKB’yi Anksiyete Bozuklukları kategorisinden çıkararak ve “Şizofreni spektrum bozuklukları” kategorisinin varlığını kabul ederek, bu iki hastalığa spektral bakış açısını desteklemiştir. Bu yazıda, ayrımı güç olan şizofreni ve OKB hastalarında tanı ve tedavi yaklaşımları gözden geçirilmiştir.

Obsessive-Compulsive Disorder (OCD) and schizophrenia are the most recognized ancient psychiatric disorders. They are markedly distinct disorders from each other due to underlying brain mechanisms, clinical manifestations and treatment strategies. However, certain demographic-clinical features and underlying pathophysiological mechanisms are common in these distinct disorders. Although the prevalence of schizophrenia and OCD is 1% and 2-3% respectively, obsessive-compulsive symptoms (OCS) are more frequent in schizophrenia than the incidental co-existence of these two disorders. Although the obsessions and delusions have distinct phenomenological features, definite differentiation between them might be difficult in some cases. Main phenomenological features that separate OCD from psychotic disorders are ego-dystonic nature of the disease and the existence of “insight” in OCD. On the other hand, the presence of “poor” insight specifier in DSM-IV about obsessions is accepted. Except this, it is also suggested that delusions might be related with obsessions and obsessions might be converted into delusions. The notifications about the presence of obsessions with psychotic content and varying extent of “insight” into the delusions in schizophrenia patients make clinical differentiation more complicated. Accordingly, an increasing amount of evidence refers a clinical spectrum from schizophrenia to OCD which is defined by the existence and extent of insight. The recent DSM manuals supported the spectral perspective to these distinct disorders by acceptance of “poor insight” specifier for OCD, exclusion of OCD from Anxiety Disorders classification and the inclusion of “Schizophrenia spectrum disorders” category. In this review, the diagnostic and treatment approaches which are difficult to differentiate in OCD and schizophrenia patients were addressed.
