Bir eğitim ve araştırma hastanesinde çalışan hemşireler arasında sigara içme, bırakma sıklığı ve sigara içme davranışının özellikleri

Amaç: Ülkemizde genel populasyonda ve sağlık personelinde sigara içme yaygınlığının gelişmiş ülkelerden daha yüksek olduğu bilinmektedir. Sağlık personeli içinde en kalabalık grubu oluşturan hemşirelerde sigara içme , bırakma sıklığı ve sigara içme davranışının özeliklerini saptamak amacıyla bu çalışma planlanmıştır.
Yöntem: Ankara Numune Eğitim ve Araştırma hastanesinde çalışan ve araştırmaya katılmayı kabul eden 250 hemşireye yarı yapılandırılmış, sigara alışkanlığı, bırakma ve sigara içme özelliklerini içeren sosyodemografik soru formu uygulanmıştır.
Bulgular: Araştırmamıza katılan hemşirelerde sigara içme oranı %58.8, bırakma oranı ise %14 idi. Sigara içme oranının boşanmış/ayrı/dul ve bekar hemşirelerde bırakma oranının ise evli hemşirelerde daha yüksek olduğu saptandı.
26-30 yaş arasında ve 2-5 yıllık hizmet süresi olan grupta sigara içme oranını diğer gruplardan daha yüksek olduğu belirlendi. Sigara içen veya halen bırakmış olan hemşirelerin % 45.1’nin 11-20 adet sigara içtiğini saptadık.
Sigara içme yılı %42.3 ile en fazla 6-10 yıl arası , günlük sigara sayısı %45.1 ile 11-20 arası olduğu belirlendi. Hemşirelerin %55’i uyandıktan 60 dakika sonra ilk sigarasını içtiği, %71.4’i sabahları çok içmediğini, %65.4’ü yemekten sonra içilen sigarasının en değerli sigarası olduğunu, %59.9’u sigara içilmeyen yerlerde zorlandığını, %27.5’i hasta iken sigara içtiğini, %56.6’sının bırakmayı deneyip bırakamadığını, %72.5’i sigara içmesini eleştirenlere kızmadığını, % 90.2’si sigara içmenin işlerini olumsuz etkilemediğini, %30.7’sinin sigarayı bırakmaya karar verdiği gün yeni paket aldığını, içenlerin %89’unun zararlarını bildiğini ve %5.5’inin sigara dışında başka madde kullandığını belirledik. Ayrıca sigara içmediği dönemde ajitasyon, dikkatsizlik, huzursuzluk, duygudurum değişiklikleri gibi yoksunluk semptomları gösterme, ve tekrar içtikten sonra bu belirtilerin iyileştiğini bildirme oranı sırasıyla %20.9, %17, %35.7, %11.5 ve %56 olduğu saptandı.
Sonuç: Hemşirelerde sigara içme davranışı önemli bir sağlık sorunudur. Ölümcül sonuçlarını bilmelerine karşın hemşirelerin yarıdan fazlası sigara içmektedir. Sigara içme oranlarını azaltmak amacıyla hemşireler sigaranın zararları ve bırakma yöntemleri konusunda daha yoğun bir biçimde eğitilmeli ve motive edilmelidir.
The Frequency of Smoking, Quitting Smoking and The Characteristics of Smoking Among Nurses of A Research and Training Hospital
Objective: In Turkey, the prevelance of cigarette smoking among general population and health professionals
is higher than the developed countries. This study is planned to determine the frequency of cigarette smoking, quitting and to evaluate the characteristics of smoking behavior among nurses that is the most crowded group of health professionals.
Method: A semi-structured socio-demographic form which was formed of questions about smoking habits, cessation and smoking characteristics was applied to 250 nurses who are working in Ankara Numune Research and Training Hospital and who gave informed consent to participate the study.
Results: Of the 250 respondents 147 are current smokers (58.8%), 26 are former smokers (%14). The rate of smoking was found highest in divorced/seperate/widow and single nurses whereas the quitting ratio was highest in married nurses. The smoking rate was highest in the group that is 26-30 [year]s old and in the group that is working for 2-5 [year]s.
We found that %45.1 of all smokers were smoking 11-20 cigarettes per day. Smoking duration of 42.3% was 6-10 [year]s. 55% of the nurses was smoking their first cigarette 60 minutes after they woke up, 71.4% was not smoking very much in the morning, 65.4% had their precious cigarette after the meals, 55.9% were having distress in smoking prohibited places, 27.5% were smoking when they were sick, 55.6% tried to quit but failed, 72.5% did not get angry when they criticized because of smoking, 90.2% did not neglect their responsibility because of smoking, 30.7% bought a pack of cigarette on the day that they decided to quit, 89% knew the harms of smoking and 5.5% were using another substance besides cigarette. The rate of agitation, loss of attention, irritability, the changes of mood during the cessation and the recovery of symptoms after starting smoking were 20.9%, 17%, 35.7%, 11.5% and 56% respectively.
Conclusion: Smoking is an important health problem among nurses. Despite the lethal consequences of smoking more than half of the nurses still continue to smoke. To decrease the rate of smoking nurses must be trained about the harms of smoking and must be motivated to quit smoking.
