Bu çalışmada İstanbul Tıp Fakültesi Psikiyatri Kliniği Duygudurum Bozuklukları Birimi'nde iki uçlu duygudurum bozukluğu tanısıyla izlenen ve 1 ila 4 yıldır koruyucu lityum tedavisindeki olgularda lityum kesilmesinin etkilerini değerlendirme amaçlanmıştır. İki yıllık izleme boyunca ortalama nüks süresi 3.28±4 aydı. Nüks oranları lityum kesiminden sonraki ilk bir ayda % 48.78, ilk 3 ay içinde % 60.97, ilk 6 ayda % 73.13 1. yıl sonunda % 83.92 ve 2. yıl sonunda % 87.80 olarak saptandı. 5 olguda (% 16.1) nüks olmadı. İlk 1 ay içindeki nükslerin yüksekliği hastalığın doğal gidişinden çok nörokimyasal bir rebound fenomeni olasılığını düşündürmektedir. Ayrıca bu sonuçlar, uzun süreli lityum koruyucu tedavisinden sonra bile lityum kesilen hastalardaki nüks riskinin yüksek olduğu ve nüks olasılığı için ilk 6-12 ayın iyi bir gösterge olduğu düşüncelerini doğrular niteliktedir.

In this study it was aimed to evaluate the effects of lithium discontinuation in patients with bipolar mood disorder which has been taken under lithium prophylaxis for 1 to 4 [year]s. The first relapses of patients following lithium discontinuation was investigated. The mean interval for relaps during two [year]s of follow up was 3.2 [month]s. The rates of relapse after discontinuation were 48.9 % for the first [month], 60.9 % for the first three [month]s and 73.1 % for six [month]s. After the first and second [year]s of discontinuation relapse rates were 83.9 % and 87.8 % respectively. Significantly higher rates of relapse in the first [month] suggests that is was due to a neurochemical rebound phenomena rather than being a result of natural course of the illness. The results show that, even after long periods of lithium prophylaxis, the risk of relapse is high after lithium discontinuation and the first 6-12 [month]s are good predictors for the possibility of relapse.
