Melankolik ve Atipik Özellikler Gösteren Depresif Bozukluklarda Serum Lipid ve Lipoprotein Düzeyleri

Amaç: Lipid profili ile depresyon ilişkisini araştıran çalışmalarda farklı sonuçlar ortaya çıkmıştır. Bu çalışmada depresif bozuklukların atipik ve melankolik özellikli klinik alt tiplerine göre serum lipid ve lipoprotein düzeylerini araştırmayı amaçlandı.
Gereç ve Yöntem: Çalışmaya 39 hasta ve 20 sağlıklı kontrol alındı. Atipik özellik (DBAÖ) ve melankolik özellik (DBMÖ) gösteren depresif bozukluklar olarak iki hasta grubu oluşturuldu. Hastalara DSM-IV için yapılandırılmış klinik görüşme formu (SCID-I) uygulanarak tanı konulduktan sonra depresyonun şiddeti Hamilton Depresyon Derecelendirme Ölçeği (HDDÖ) ile değerlendirildi. Hasta ve kontrollerde vücut kitle indeksi (VKİ) ölçümüyle beraber 12 saatlik açlık sonrası sabah 08.00-09.00 saatlerinde serum total kolesterol (TK), trigliserit (TG), yüksek dansiteli lipoprotein (HDL), düşük dansiteli lipoprotein (LDL) ve çok düşük dansiteli lipoprotein (VLDL) değerlendirildi. TK/HDL ve LDL/HDL oranları hesaplandı.
Bulgular: Hasta grupları ve kontrol grubu VKI, yaş ve cinsiyet bakımından eşleştirildi. DBAÖ ve DBMÖ grupları arasında ve tek tek hasta gruplarının kontrol grubuyla karşılaştırılmasında, serum TK, HDL, LDL, VLDL, TG düzeyleri ve TK/HDL ve LDL/HDL oranları bakımından istatistiksel olarak herhangi bir anlamlı fark yoktu (her biri için P>0.05). Yine üç grupta da serum lipid ve lipoprotein düzeyleri ile cinsiyet arasında hiçbir korelasyon bulunmadı.
Sonuç: Lipid profilinin melankolik özelliklerle atipik özelliklerin ayrımında kullanılabilecek bir biyolojik belirteç olduğu hipotezini ve cinsiyetin depresyonda lipid profilini etkilediği görüşünü bizim sonuçlarımız desteklememiştir.
Serum Lipid and Lipoprotein Levels in Depressive Patients with Atypical and Melancholic Features
Objective: The studies in which the relationship between serum lipid profile and depression was examined had revealed controversial results. The purpose of the present study was to explore the relationship between serum lipid and lipoprotein levels and depressive disorders with atypical and melancholic features.
Method: The study consisted of 39 depressive patients and 20 healthy controls. The depressive patients were divided into two groups, depressive disorders with atypical features and melancholic features. The patients were assessed by Structured Clinical Interview for DSM-IV and then, Hamilton Depression Rating Scale (HDRS) was administered to all patients. Fasting serum total cholesterol (TC), triglyceride (TG), high density lipoprotein (HDL), low density lipoprotein (LDL) and very low density lipoprotein (VLDL) levels were measured at 08.00–09.00 a.m. TK/HDL, LDL/HDL and body mass index (BMI) was calculated for all subjects.
Result: The patient groups and control group were matched for sex, age and BMI. We did not find any statistically significant difference in serum TC, HDL, LDL, VLDL, TG levels and TK/HDL and LDL/HDL among groups (P>0.05). There was also no correlation between serum lipid and lipoprotein levels and sex in patients and control groups.
Conclusion: Our results did not support the hypotheses that lipid profile was a biological marker to distunguish depressive disorder with melancholic features from atypical features, and that there may be a gender difference between lipid concentrations and major depression.
