Ağız Yanması Sendromu (AYS), hastalar tarafından ağız mukozası ve peri-oral bölgede genelde yanma veya ağrı şeklinde
tanımlanan belirtiler kompleksidir. Bu durum da AYS’de psikolojik kökenli hastalıkları akla getirir. Burada menapoz sonrası
kadınlar, depresyon ve kaygı önemli bir yer tutar.
Olgu 59 yaşında kadın hasta idi. Yakınmaları 14 sene önce damağından boğazına kadar yanma ile başlamıştı. Hastanın yapılan
klinik muayenesinde lokal ya da sistemik organik bir neden saptanamadı. Yapılan ruhsal değerlendirmesinde ise, yanmanın
ortaya çıkışının geçmişteki yaşam olaylarıyla ilişkili olması, ailede kronik hastalık ve şiddet öyküsünün olması, inkarın kullanılması,
kaygının gözlenmesi ve Zung Depresyon Ölçeği’nde orta düzeyde (SDS=65) depresyon saptanması nedenleriyle hastaya
AYS tanısı konuldu.
Hastaya antidepresan olarak, seçici serotonin gerialım önleyicisi olan sertralin 50 mgr/gün dozunda başlandı. Buna ilaveten
hastaya bireysel ve bilişsel-davranışçı tedavi uygulandı. Kontrol muayenesinde şikayetlerinin azaldığı saptandı.
Bu çalışmada AYS tablosu sergileyen bir hastanın somatoform bozuklukla ilişkisi belirtilerek bu ilişkinin literatür ışığında
tartışılması amaçlanmıştır. AYS’nin, erken dönemde multidisipliner bir yaklaşımla ele alınarak, erken teşhis ve tedavisi gereken
bir hastalık olduğu düşüncesindeyiz.

The Burning Mouth Syndrome (BMS) is the complex of the symptoms, which are generally defined by patients as burning or
pain in mouth and peri-oral region. From this point of view, it reminds the disorders which are directly related to psychology or
the illnesses at the basis of psychology such as menopause, depression, and anxiety.
The case was a female patient who was 59 [year]s old. The complaints of the patient had started with burning from palate to
throat 14 [year]s before. A local or systemic organic reason couldn’t be found clinically in the examination of the patient. In the
mental evaluation, the patient was diagnosed as BMS because of the following: being related with coming out of burning of the
life events in the past, her family having the history of a chronic illness and abuse/violence, the patient denying her illness, the
patient having anxiety and patient having depression the level of which was at the middle level (SDS=65) according to Zung
Depression Scale.
The patient was given a antidepressant agent called sertralin, which is a selective serotonin re-uptake inhibitor, in a dose of 50
mg/day. In addition to this, the patient was treated by individual and cognitional-behavioural therapy. In the control
examination, she reported that her complaints had diminished.
In this case report, it was aimed to discuss a patient with BMS related to somatoform disorder in the light of literature. We
suggest that a case with BMS requires early diagnosis and treatment and also should be dealed with a multidiscipliner
approach
