Hastanın yardım almak için başvurduğu bir terapistle kurduğu ilişkinin cinsel tacize dönüşmesi hem hasta, hemde terapist için, hemen her zaman yıkıcı durumdur. Ülkemizde bu konu yeterince ciddiye alınmamakta, açıkça tartışılmamakta, bu tür durumlarda yeterli yaptırım uygulanmamakta, hastalar ise genellikle şikayetçi olmaktan çekinmektedirler. Böyle bir ilişkinin temelinde hem terapist hem de hasta ile ilgili kişilik özelliklerinin karmaşık bir etkileşimi yatar. Terapist-hasta ilişkisinde cinsel sınırların delinmesine giden yolun, terapötik ilkelerin en zararsız görünenlerinden başlayarak çignenmesi ile açıldığı bildirilmektedir. Terapist terapötik ilkelere bağlı kalmakla, psikodinamik yönelimli terapi uyguluyorsa süreç içinde oluşabilecek erotik aktarımın altında yatan çatışmaları anlamak ve hastaya anlatmakla, geçmişle ilintisini göstermekle bu arada kendisinde de oluşan duyguları tanımak ve anlamakla yükümlüdür. Eğitimin, önemli olmakla birlikte, bu tür davranışları engellemekte yeterli olmadığı, özellikle ağır kişilik bozukluğu tanısı konmuş terapistlerin rehabilitasyonlarının güç olduğu, yaptırım uygulamak gerektiği bildirilmektedir. Yazıda, erotik aktarımın, terapistin hastanın beklentilerine uyması sonucunda cinsel tacize dönüştüğünde hasta için nasıl yıkıcı olabileceğini sergilemek amacıyla bipolar bozukluk Tip III ve sınır kişlik bozukluğu tanısı konmuş bir olgu sunulmuş, ardından da böyle bir ilişkinin dinamikleri genel olarak özetlenmiştir

Sexual abuse of the patient is always harmful for the patient and destructive for the therapist. In our country, despite legal rules, this unethical behaviour is not considered to be serious enough, is not discussed sufficiently and the patients choose not to use. The dynamics of such a relationship is complex, depending on the characteristics of both parties. The abused patients are reported to be hostile individuals with borderline dynamics, anxious attachments and have been abused as a child. The abuser is usually a male therapist mostly with countertransference problems. Some of the therapists may have serious narcissistic and antisocial psychopathologies, and may even be psychotic. It is accepted that the first steps to sexual abuse usually start with minor, seemingly harmless violations of therapeutic boundaries. It is the therapist’s task not to violate boundaries, to understand his own feelings as well as the patient’s and to show his patient the actual conflict behind the erotic transference and its relation with the past if the therapy is a psychodynamic one. While most of the sexual abuse incidents are reported to be related with transferential issues, education of the therapist alone does not seem to resolve this ethical problem. The rehabilitation of the therapists with serious personality disorders has been reported to be difficult. In this article, a case of a female patient diagnosed as bipolar disorder who has been abused sexually by a male therapist is summarised to show how destructive the therapist’s acting out behaviour could be, almost leading her to commit suicide, and the general dynamics of such a relation is reviewed.
