Panik Bozukluğunda Atakların Klinik Özellikleri: Q Yöntemi İle Bir Çalışma

Amaç: Bu çalışmanın amacı Q yöntemi kullanılarak panik bozukluğun alttiplerini tanılamaktır.
Yöntem: Veriler DSM-IV ölçütlerine göre panik bozukluğu tanısı almış 48 hastadan toplanmıştır. Her hasta son tipik panik atağını gözönünde bulundurarak, 19 başlıktan oluşan listeden seçim ve sıralama yapmıştır.
Bulgular: Q analizi sonrası dört grup saptanmıştır: 1) Kardiyovasküler tip, 2) Gastrointestinal ve vestibüler tip, 3) Solunumsal tip, 4) Bilişsel tip. Gastrointestinal ve vestibüler tipte ayrıca yükseklikle ilgili (uçak korkusu, asansör korkusu) korkuların varlığı dikkati çekmiştir. Solunumsal alttipte kadınların çoğunlukta olduğu ve daha çok agorafobik belirtiler görüldüğü, ayrıca bilişsel alttipte diğer alttipte yer alan kişilere göre daha yoğun beklenti anksiyetesi bulunmuştur.
Sonuç: Panik bozukluğu tanısı alan kişilerin panik ataklarının, belirtilerin önceliği açısından farklılık gösterdiğine ilişkin bulgular saptanmıştır. Tanımlanan alttiplerden bir bölümü bu konuda daha önce yapılan çalışmalarda da gösterilmiştir. Boğazında düğümlenme, konuşulanları duyup yanıt verememe, zihin bulanıklığı gibi DSM-IV’de bulunmayan belirtilerin sıklıkla bildirilmesi bu konunun daha çok araştırılması gerektiğini düşündürmüştür.
Clinical Characteristics of Attacks in Panic Disorder: A Study With Q Methodology
Objective: This study aimed to describe subtypes of panic disorder, using Q methodology.
Method: The data were collected from 48 patients who were diagnosed as panic disorder by DSM-IV criteria. Each patient ranked and sorted 19 items considering her/his last typical panic attack.
Results: In this study, four subtypes of panic disorder were determined according to the dominating symptoms in the patients’ descriptions: 1) Cardiovascular type, 2) Gastrointestinal and vestibular type, 3) Respiratory type, 4) Cognitive type.
Conclusion: The majority of patients with acute history were suffering from classical panic attacks; the patients with gastrointestinal and vestibuler symptoms were also suffering from phobias related to heights (flying-phobia, elevator-phobia). There were female predominance and more agoraphobic patients in the respiratory group; also more patients were suffering from anticipatory anxiety in the cognitive group than other groups. The type of attacks influences the patient’s admission style, spontaneous panic attack frequency, the types of phobia. In our group items like “mental confusion”, obstruction feeling in the throat” and “being unable to respond while hearing the talk” which don’t exist in DSM-IV were also endorsed by our subjects. The interpretation of the findings suggest the need for more structured studies. In subtyping studies the biological vulnerability factors should be studied in detail in the context of system specificity.
