Erişkinlerde dikkat eksikliği ve hiperaktivite bozukluğu tedavisinde psikoterapi ve psikososyal yaklaşımlar

Dikkat Eksikliği Hiperaktivite Bozukluğu (DEHB) tedavi edilmediğinde bireysel ve toplumsal olarak maliyeti yüksek bir bozukluktur ve DEHB’nin tedavisinde tüm alanlara yönelik iyileştirici girişimler koruyucu ruh sağlığı hizmetleri açısından anlam taşıyacaktır. DEHB’nin erişkinlik döneminde tedavisi ilk sıra tedavisi olarak önerilen farmakoterapi mutlaka psikoeğitim, danışmanlık, çevresel yapılandırma ve psikososyal girişimleri de içeren kapsamlı bir tedavinin parçası olmalıdır. DEHB’nin tedavisi için farmakoterapi vazgeçilmez bir unsur olsa da mutlaka psikosoyal girişimler ve psikoterapi ile kombine edilmelidir. Psikoterapi farmakoterapi sonrasında kalan tortu belirtilerin silinmesinde önem taşır. Özellikle psikostimulanlar veya stimülan olmayan atomoksetin, bupropion gibi tedavilere yanıt vermeyen olgularda, ilaç
kullanmayı istemeyen bireylerin tedavisi ve takibinde psikoterapinin rolü öne çıkmaktadır. Psikoeğitim, destekleyici psikoterapiler ve bilişsel davranışçı terapiler ile başlayan çalışmalar zaman içinde farkındalık temelli bilişsel terapiler ve kılavuzluk (coaching) yaklaşımlarını da içerecek şekilde çeşitlenmiştir.
Psychotherapy and psychosocial interventions in the treatment of adult attention deficit hyperactivity disorder
The failure of treatment of adult attention deficit hyperactivity disorder (ADHD) has an important cost on individual and social life. The psychotherapy should be a part of a comprehensive treatment. Effective intervention including psychotherapy could have protective effect. The first–line treatment of adult ADHD with pharmacotherapy should be expanded to a comprehensive treatment plan including psychoeducation, counselling, environmental modulation and psychosocial interventions. The pharmacotherapy of adult ADHD is a critical step in treatment plan; whereas it must be combined with psychotherapy and psychosocial interventions. Psychotherapy is important in the management of residual symptoms retaining after pharmacotherapy. The role of psychotherapy comes into prominence in the treatment and
management of especially cases not responding to psychostimulants or non-stimulants such as atomoxetin and bupropion, patients who do not approve for pharmacotherapy. Psychoeducation, supportive psychotherapies and cognitive behavioural therapies supply clinicians a broad psychotherapy concept, evolving into mindfulness based cognitive therapies and coaching.
