Amaç: Bu çalışmada depresif bir ataktan iyileşen bir grup hastada iyi kalma süresi, alevlenme ve yineleme oranları ile ortaya çıkaran risk etkenlerinin belirlenmesi amaçlanmıştır. Yöntem: DSM-III-R ölçütlerine göre bipolar bozukluğun depresif epizodu dışında depresif bozukluk tanılarından birini alan, akut tedavi sonucu iyileşen 48 hasta 2 yıl boyunca izlenmişlerdir. Kümülatif iyi kalma olasılığı sağkalım analizi ile değerlendirilmiştir. Yineleme olasılığını arttıran risk faktörlerinin saptanması amacıyla, izleme sırasında yineleme gösteren ve göstermeyen grup sosyo-demografık veriler, hastalık öyküsü, başvuru sırasındaki depresyon atağının özellikleri, biyolojik değişkenler ve psikososyal stresörler açısından değerlendirilmiştir. Bulgular: İyileştikten sonra iyilik halini sürdürme olasılığı ilk 6 ayda %86.7, 12 ayda % 75.6, 24 ayın sonunda ise % 46.3 olmuştur. Atak sayısı (özellikle 3 atak ve fazlası), ikincil depresyon olması, stresör varlığı, sürdürüm tedavisinde düzenli ilaç kullanımının olmaması iyilik durumunun sürdürülmesini olumsuz yönde etkilemektedir Tartışma: Depresyonda artan atak sayısıyla birlikte hastalığın yineleme riski artmaktadır. Bu nedenle özellikle atak sayısının fazla olduğu, diğer risk etkenlerinin bulunduğu hastalarda uzun süreli koruyucu tedavi düşünülmelidir.

Objective: The objective of this study is to evaluate the probability of remaining well, relapse and recurrence after recovery from an episode of major depression and the factors affecting the risk of recurrence. Method: 48 patients who had recovered from an episode of major depressive disorder according to DSM-III-R criteria, excluding bipolar disorder, depressive episode were evaluated for 2 years. The probability of remaining well after the index episode were assessed by survival function. We evaluated socio-demographic characteristics, psycho-social stressors, the course of index episode and biological variables in two groups in order to assess the factors that increase the risk of recurrence. Results: The probability of remaining well after recovery from index episode was %86.7 at 6 months, %75.6 at 1 year and %46.3 at 2 years. The number of episodes (especially three ormore episodes), comorbidity with other psychiatric disorders, presence of stressors and shortness of continuation treatment duration were the factors affecting the risk of recurrence. Conclusions: The risk of future episodes increases as the number of past episodes increases. Prophylactic drug treatment should be suggested especially for patients with three or more episodes and for those who have other risk factors.
