Transvestik fetişizm; heteroseksüel kişinin en az 6 ay boyunca, cinsel uyarılma, masturbasyon veya cinsel ilişki için, cinsel yönden uyarıcı fantezi olarak diğer cinse ait giysileri kullanması ile tekrarlayan şekilde ortaya çıkan parafilik bir durumdur (DSM IV). Neredeyse her zaman erkeklerde rastlanılan bu sıra dışı cinsel eylem, sıklıkla çocukluk veya puberte çağında başlar. Kişinin cinsel kimliği ile ilgili herhangi bir şüphesi yoktur ve cinsel yönelimi de heteroseksüeldir. Özellikle transseksüalizm ile ayırıcı tanısı yapılmalıdır.

Sunulan olgu 36 yaşında evli erkektir. Mahkemesince, Adli Tıp Kurumu 4.Adli Tıp İhtisas Kurulu'na (4.ATİK) , 08.05.2002 tarihinde e vden takı ve kadına ait dantelli iç çamaşırları çalma iddiası ile ceza ehliyeti sorulmuştur. Muayene bulguları ve uygulanan Rorschach projektif testleri sonucunda kadın iç çamaşırı ve takı çalma suçu cinsel uyarıyı sağlamaya yönelik bir eylem olarak değerlendirilerek DSM IV kriterlerine göre transvestik fetişizm tanısı konulmuştur.

After clinical examination and Rorschach test, transvestic fetishism was diagnosed according to DSM IV criteria.
