Şizofrenide Ruhsal ve Toplumsal Beceri Eğitimi Uygulaması Grup Eğitimi Deneyimleri

Amaç: Şizofrenide ruhsal ve toplumsal beceri eğitimi (ŞRTBE) programı, alandaki temel kaynaklara dayanarak hastaların ruhsal ve toplumsal becerilerini geliştirmeye yönelik olarak hazırlanmış bir programdır. Programda aile eğitimi de verilerek hastalann ruhsal ve toplumsal becerilerini geliştirmesinde ailelerin önemli birer yardımcı olmaları sağlanmaktadır. Bu yazıda ŞRTBE programının Kocaeli Üniversitesi Psikiyatri Bölümü'ndeki uygulama örneklerini ve sonuçlarını sunmak ve programı tanıtmak amaçlanmıştır.
Yöntem: Polikliniğe başvuru sırasına göre programa katılmayı onaylayan şizofrenili hastalar 6 ya da 7 kişilik gruplar halinde ŞRTBE programına alındılar. Grup eğitimi haftada bir kez yaklaşık 6-8 aylık sürede tamamlandıktan sonra üyeler destek oturumlarına devam ettiler. Hasta gruplarıyla eşzamanlı olarak aileler de iki haftada bir aile-eğitimi gruplarına katıldılar. Programın hastalar üzerindeki etkisi, hastaların ilk ve son oturumda doldurdukları ilk ve son testler arasındaki fark incelenerek ve programdan sonraki ay içerisinde aileler tarafından doldurulan değerlendirme formu ile değerlendirildi.
Bulgular: Dört grup eğitimi başlatıldı, ilk grupta hasta sayısının 9 oturum sonra yediden ikiye düşmesi üzerine bu grup kapatıldı, gruba devam eden 2 üye sonraki gruba dahil oldu. Üç grup eğitimi tamamladı. Toplam 25 şizofreni hastasının katıldığı eğitimi tamamlayan hasta sayısı 19 (%76.0) idi. içgörü azlığı, psikotik alevlenme, gövde distonisi ve ulaşım sorunları grubu bırakma nedenleri arasında yer almaktaydı. Eğitimi tamamlayan hastaların gruplara devam oranı ise %80.7 olarak gerçekleşti. Tüm hastalar eğitimden sonra destek oturumlarına devam ettiler. Hastaların eğitimin başında ve sonunda doldurdukları ilk ve son testlerden aldıkları puan ortalamalarının karşılaştırmasında %14.3'lük bir bilgi artışı oldu. Ailelerden alınan geri bildirim sonuçlarına göre hastalar iletişim, toplumsallaşma, ilaç tedavisi ve hastalık belirtileri alanlarında daha becerili oldular.
Sonuç: Uygulanmasında herhangi bir sorunla karşılaşılmayan ve aile eğitimini de içine alan ŞRTBE programı şizofreninin tedavisinde ruhsal ve toplumsal rehabilitasyon için önemli bir araç olabilir
The Practice of Mental and Social Skills Training in Schizophrenia: Experince from Group Therapy
Objective: Psychosocial skills training (PSST) program in schizophrenia was prepared to improve the patients' psychological and social skills on the basis of the main sources in this field. Families play an important role for the patients to develop their psychological and social skills by participating in the educational program. The aim of this article is to present the practice and results of PSST program in psychiatric outpatient clinic at the University of Kocaeli in Turkey.
Method: Twenty five consecutive patients with schizophrenia from the outpatient clinic who accepted to participate the PSST education program were enrolled in the study. Patients were divided into four groups (each consisted of 6 or 7 individuals) whom had weekly education for six or eight [month]s. Patients' families also received psychoeducation in accordance with the patients' group in every two-week. Patients' groups continued to participate booster sessions after completing the program. The effects of the PSST program were assessed by comparing both the pre and post-test scores filled by the patients and the Evaluation Form filled by family members after the cessation of program.
Results: Four groups were started consecutively. The first group was cancelled due to the drop-out of 5 patients throughout 9 sessions. Two members remaining from the first group participated in the second group. Three of the four groups was able to complete the program. 19 patients of all those 25 (76.0%) completed the program. Lower level of insight, psychotic exacerbation, trunk distonia, and transportation problems were among the reasons of drop out. The rate of continuity was 80.7% for the patients for those who completed the program. All patients continued to participate to booster sessions after the education program. Patients' level of knowledge about the illness and its treatment improved (14.3%) according to the pre and post-test scores. Patients developed skills in the fields of communication, socialization, medication management, and symptom management according to the family's reports.
Conclusion: PSST program including family education, without any problem in practicing, could be an important tool for psychosocial rehabilitation in the treatment of schizophrenia.
