Epileptik psikoz: İki olgu raporu

Amaç: Epilepsili hastalarda epileptik olmayan hastalara ve temporal lob parsiyel epilepsili hastalarda ise jeneralize epilepsili hastalara göre psikozların daha sık görüldüğü bildirilmiştir. Sol temporal lob odaklı temporal lob epilepsili hastalarda 'şizofreni benzeri psikoz' gelişme riskinde artış bulunmuştur. Epilepside psikozlar interiktal, iktal ve postiktal evrede ortaya çıkabilir. Bu yazıda Genel Tıbbi Duruma (Epilepsi) Bağlı Psikotik Bozukluk tanısı konulan iki olgu sunulmuştur.
Olgu Raporları: İlk olgu otuz yaşında, erkek, onyedi yıldır kompleks parsiyel nitelikte epileptik nöbetler tanımlanan, EEG'de sol temporal lob odağı tespit edilen, son yedi yıldır nöbetlerin olmadığı dönemlerde içe kapanma, dinsel uğraşlarda artış, şüphecilik, kötülük görme düşünceleri bulunan ve 100 mg/gün Lamotrijin tedavisiyle nöbetleri ve düşünce bozuklukları belirgin şekilde düzelen bir interiktal psikoz olgusudur. İkinci olgu ise kırkbeş yaşında, erkek, yirmi yıldır kompleks parsiyel tarzda epileptik nöbetler tanımlanan, EEG'de sağ ön ve orta temporal bölgelerde keskin dalganın odaklaştığı epileptiform aktivitesi bulunan, nöbet sonrası dönemlerde dini konularla ilgili uğraşlarda artma, kendisinin Allah veya peygamber olduğunu öne sürme gibi düşünceleri olan ve 600 mg/gün Karbamazepin tedavisiyle psikotik tablosu düzelen bir postiktal psikoz olgusudur.
Sonuç: Bu iki olgu nedeniyle epileptik psikozlar genel olarak gözden geçirilmiş ve sonuç olarak epileptik psikozlu hastaların gerek kendilerinin gerekse ailelerinin yaşam kalitelerini düzeltmek amacıyla doğru tanı konulmasının önemli olduğu ve bu hastaların epileptik nöbetlerinin düzeltilmesi ile psikotik tablolarının da tedavi edilebileceğinin unutulmaması gerektiği kanısına varılmıştır.
Epileptic Psychosis: Report Of Two Cases
Purpose: It was reported that psychosis is seen more frequently in epileptic patients than non-epileptics and in partial temporal lobe epileptics than generalised epileptics. The development of schizophrenia-like psychosis risk in the left temporal lobe focused temporal lobe epileptic patients is found to be increased. Psychosis in epilepsy could be occur in interictal, ictal and postictal periods. In this report we discuss two patients with psychotic disorder due to the general medical condition (epilepsy).
Case Reports: The first case is a 30 [year]s old man who have complex partial epileptic attacks for 17 [year]s. A left temporal lobe focus was found by EEG examination. For the past 7 [year]s he was having social withdrawal, an increase in religious occupation, doubtness and persecution ideas in interictal periods. In this interictal psychosis case, epileptic attacks and thougt disorders were improved by daily 100 mg Lamotrigine treatment. The second case is a 45 [year]s old man. He has complex partial epileptic atttacks for 20 [year]s. Epileptiform activity with sharp waves focused right anterior and middle temporal region was found on EEG examination. He has delusional ideas such as is god and prophet in postictal periods. Psychotic symptoms were improved by daily 600 mg Carbamazepine treatment.
Conclusion: In this study two cases are reported and literature is rewieved. We concluded that accurate diagnosis is essential for improving the life qualities both patients and their families. Therefore one must bare in mind that the treatment of psychotic symptoms in those patients is possible by improving the epileptic attacks.
