Şizofrenide Depresyon ve Anksiyete Bozuklukları Eştanısı: Sosyodemografik ve Klinik Değişkenler ile İlişkisi

Amaç: Bu çalışmada, bir grup şizofreni hastasında DSM-IV-TR tanı ölçütlerine göre eşlik eden depresyon ve anksiyete bozukluklarının belirlenmesi ve bu bozuklukların sosyodemografik ve klinik değişkenlerle ilişkisinin ortaya çıkarılması hedeflenmiştir.
Yöntem: Sağlık Bakanlığı Dışkapı Yıldırım Beyazıt Eğitim ve Araştırma Hastanesi Psikiyatri Kliniği’ne başvuran ve DSM-IV-TR’ye göre şizofreni tanısı almış olan 78 hasta çalışmaya alındı. Tüm hastalara sosyodemografik bilgi formu, DSM-IV Eksen I Bozuklukları İçin Yapılandırılmış Klinik Görüşme (SCID-I), Pozitif Semptomları Değerlendirme Ölçeği (SAPS) ve Negatif Semptomları Değerlendirme Ölçeği (SANS) uygulandı.
Bulgular: 78 hastanın %32.1’i (n:25) majör depresyon, %6.4’ü (n:5) obsesif-kompülsif bozukluk %3.8’i (n:3) eşik altı obsesif-kompülsif belirtiler, %2.6’sı (n:2) yaygın anksiyete bozukluğu, %2.6’sı (n:2) sosyal fobi, %2.6’sı (n:2) travma sonrası stres bozukluğu ve %1.3’ü (n:1) başka türlü adlandırılamayan anksiyete bozukluğu eştanısına sahipti. Ek majör depresyonu olanlarda eğitim düzeyi daha düşük (p<0.05), intihar oranları daha yüksek (p<0.05) bulundu. Anksiyete bozukluğu eş tanısı ile sosyodemografik ve klinik değişkenler bir ilişki bulunmadı.
Tartışma ve Sonuç: Şizofrenide ek depresyon ve anksiyete bozuklukları önemli oranlarda görülmekte ve eştanı hastalığın gidişini olumsuz etkileyebilmektedir. Yapılandırılmış tanı görüşmeleri, yetersiz eştanı konulmasının önüne geçebilir. Şizofrenide anksiyete ve depresyonun etiyopatogenezini ve prognoza etkilerini anlamak ve tedavi seçeneklerinin etkilerini değerlendirmek için ileri çalışmalara gereksinim vardır.
Comorbidity of Depression and Anxiety Disorders in Schizophrenia: Relationship with Sociodemographic and Clinical Variables
Objective: In this study we aimed to identify depressive and anxiety disorders in a group of schizophrenia patients diagnosed according to DSM-IV-TR criteria and to investigate their relationship with sociodemographic and clinical variables.
Methods: Seventy eight patients diagnosed as schizophrenia according to DSM-IV-TR criteria were recruited from Diskapi Yildirim Beyazit Education and Research Hospital. Sociodemographic data form, Structured Interview for the Diagnostic and Statistical Manual of Mental Disorders-Fourth Edition (SCID-I), Scale for the Assessment of Positive Symptoms (SAPS) and Scale for the Assessment of Negative Symptoms (SANS) were applied to participants.
Results: Among seventy eight patients, 32.1% (n:25) had major depression, 6.4% (n:5) had obsessive-compulsive disorder, 3.8% (n:3) had obsessive-compulsive symptoms, 2.6% (n:2), had generalized anxiety disorder, 2.6% (n:2) had social phobia, 2.6% (n:2) had posttraumatic stress disorder and 1.3% (n:1) had anxiety disorder otherwise unspecified as comorbid disorder. Patients with comorbid major depression had lower educational levels (p<0.05) and higher suicide rates (p<0.05) compared to other patients. No relationship was found between comorbid anxiety disorder and sociodemograpic and clinical variables.
Discussion and Conclusions: Comorbid depression and anxiety disorders may be seen at important rates in schizophrenia and this comorbidity may adversely affect the prognosis of the disease. Structured diagnostic tools may prevent underdiagnosis of comorbid disorders. Further research is needed to understand the etiopathogenesis of the comorbidities and their effect on prognosis of schizophrenia and to assess the effect of treatment choices.
