Psikiyatrik hastalıkların tedavisinde uygun seçenekler uygulansa bile bazen istenilen düzeyde iyileşme sağlanamamaktadır. Obsesif kompulsif bozuklukta (OKB) ilk basamakta önerilen ilaçlara %60'a varan bir yanıtsızlık gözlenmektedir. OKB'de tedavi etkinliğini değerlendirmede en önemli güçlükler hastalığın farklı semptomlarla seyredebilmesi ve komorbid durumların gözlenebilmesidir. Günümüzde tedaviye dirençli olarak değerlendirilen olgularda değişik stratejilerin uygulanması önerilmektedir. Farmakolojik olarak, ilaçların değişimi, güçlendirilmeleri, intravenöz verilmesi gibi yöntemler denenebilir. İlaç dışındaki diğer tedavi seçenekleri, bilişsel davranışçı terapi, aile eğitimi, transkraniyal manyetik uyarım ve nörocerrahidir. Bu tedavi yöntemlerinin uygulanmasına rağmen hala bazı hastalarda istenen iyileşmemenin olmaması, OKB'nin etyolojisi ve OKB'de tedaviye direnç ile ilgili daha fazla çalışma yapılması gerektiğini ortaya koymaktadır.

The desired level of improvement sometimes could not be provided in the treatment of psychiatric disorders even if the suitable options were executed. An unresponsiveness to the drugs which are recommended as first-line in obsessive-compulsive disorder (OCD) is observed up to 60%. The most important challenges in the evaluation of treatment efficacy of OCD are the presence of the disorder with different symptoms and observation of the comorbid situtiations. Today, implementation of different strategies are recommended in patients evaluated as treatment-resistant. Pharmaceutically, methods such as switch, augmentation or intravenous administration of drugs can be tried. Treatment options other than drug therapy are cognitive behavioral therapy, family education, transcranial magnetic stimulation and neurosurgery. Although the implementation of these therapeutic procedures, still, the absence of the desired improvement in some patients reveal that more study needs to be done about the etiology of OCD and treatment resistance in OCD.
