Ruhsal hastalığı olan gebe/gebe kalmayı planlayan kadına yaklașım

Ruhsal hastalığı olan ve gebe kalmayı planlayan ya da gebe kadın hasta hekim için pek çok sorunu taşımaktadır: Tedavi etmek ya da tedavisiz
bırakmak? Bu yazıda psikiyatristlere bu hastaların tedavisinde karar verme süreçleri ile ilgili kimi ilkeler ve öneriler sunulacaktır. Gebelik sırasında
ilaç tedavisi ile ilgili korkular ve kaygılar hem hasta, hem hasta yakınları hem de hekim için karmaşık problemdir. Genellikle hasta ve
hasta yakınları gebelik sırasındaki ruhsal hastalığın tedavi edilmemesinin anne ve fetus üzerine olan olumsuz etkilerini göz ardı eder. Bütün
bu durumlar hasta, hasta yakını ve hekim arasında tedavide karar verme süreçlerinin paylaşılmasının önemini vurgular.
Konunun uzmanları gebelik sırasında major depresyon tedavisi için tedavi kılavuzu sunmuşlardır; diğer ruhsal hastalıkların gebelik sırasında
tedavisi için benzer karar verme süreçlerini kullanabiliriz. Değerlendirmedeki ilk basamak hastalığın formülasyonu (şimdiki ve geçmiş
hastalık tanısı, ektanılar, özkıyım ve kendine zarar verme davranışları, şimdiki ve önceki psikoterapi ve ilaç tedavisi, tedaviye uyumu); risk
etmenlerini belirleme, literatür gözden geçirme, tedavi seçeneklerini değerlendirme, tedavinin risk ve yararlarını tartma ve bilgilendirilmiş
onam ve dokümantasyon. Tedavi yaklaşımlarının risk ve yararları ayrıntılı olarak hasta dosyasına kayıt edilmelidir.
Management of women with mental disorder planning pregnancy or who is pregnant
Pregnant women and/or pregnancy planning women with mental disorders present clinicians with a range of clinical challenges: treating
the illness or not treating? This article provides psychiatrists some principles and suggestions how to handle the decision making process
during the treatment of these patients. The fears and worries about the risk of drug treatment during pregnancy for the patient, the family
members and clinician are a complex problem. The patient and the family members usuallyignore the adverse effects of untreated mental
illness during pregnancy to the mother and fetus. These reinforce the importance of making treatment decisions shared between the patient,
family members and the clinician.
The experts provide guidelines for the treatment of major depression during pregnancy, we can apply the decision-making process to the
treatment of other mental disorders. The first step of assessment is the formulation of the illness (current and past diagnosis, comorbidity,
suicide/self-destruction, current and past psychotherapy and drug treatment, treatment adherence); then description of risk factors, the
review of literature for the optimal treatment of mental disorder during pregnancy, the evaluation of treatment options, weighing the risksbenefits
of the treatment and not treating the disorder for the mother and the fetus, and the documentation of the assessment. A discussion
of the risks and benefits of all interventions should be carefully documented in the patient’s chart.
